Before I really get into the process of approval, I've been working with my mother (who has had gastric bypass) to figure out if my conditions are considered weight related. Specifically back problems, and the risks of being obese with my malformed heart. I don't have high blood pressure (it's abnormally low due to my heart), or many of the other weight related issues commonly listed. In your guys' experience, will the back problems and the risk with my heart be enough?

Those are not listed as the usual things they look for...I'm not sure about your heart...they are looking for things the surgery will more likely improve. I have back problems and I definitely think it will help me, but I haven't found out yet if it will count"

Most insurance companies have a list of what they consider to be co-morbidities, and it varies both by insurance company and your specific plan. They should be able to provide you with the list if it isn't in your member handbook. And if they don't have a set list, just make sure your surgeon and PCP are on board with backing up your conditions as being weight-related. Good luck!
