You should be able to find out the coverage percentage also. Otherwise, they will pay a percentage as specified by your policy, subject to your outstanding deductible. There are surgeon's fees, hospital fees, fees for the program at the surgeon plus perhaps lab fees and other misc fees and your copay to consider. Your surgeon's office will call the insurer to verify your coverage and terms and the insurer's program requirements, but you should also call the benefits department to to double check. (my surgeon's insurance admin screwed up and I had to go through an appeal after being denied.) Not all insurers or policies require a 6 month supervised diet. Even for one insurance company, say UHC, it may depend on whether you have an HMO or PPO or whether your employer bought bariatric coverage as part of your plan. Hope that helps.
