Kat817 19 Posted February 26, 2007 Your Dr.'s office should have all the info they need for the letter from your file. Some offices charge for filling out any forms---others fill them out, write letters, whatever, as part of your care. My PCP, wrote several letters for me, all at no charge. He always made sure I got a copy of anything he sent. I will say after the first one, I truly wished he would let me write it and him sign it---he was short and concise, I felt he should have detailed more----but it is in the past now!!! niecyrenee 2 Posted February 26, 2007 I just asked my doctor to write me a recommendation letter and he did, free of charge. I gave him an idea of what they were looking for and he put it all in the letter and gave me a copy. It did the job cause not long afterwards, I was approved!
