6 month diet paperwork

Does anyone know where I can get some examples of what needs to be written on these monthly diet forms? I was given these forms and told by my surgeon to have my pcp fill out once a month what the doctor recommended, the diet plan we discussed, etc, and my doctor's been writing things like "eat less calories" "walk 45 minutes a week" - one line type stuff. I'm scared that my insurance company will decline me if I have this type of stuff on my paperwork. Anyone have any tips for me? :) thanks!

Hi Slowtogo: i dont think I do. From what I understand, I just have to visit my PCP for six monthly visits, then submit that paperwork to my surgeon. I had to do a psych evaluation too, but that was completed in August.

When I fill them out for patients they typically are similar....I'll do things like 'add 5 minutes of walking twice daily and decrease simple carbs in diet, increase Protein to at least 60 grams daily'. I have never had an issue with patients getting approved. I also give the patient some type of computer generated print out on diet and/or exercise. I think ypthe insurance co just want to know you were there and something was discussed weight loss wise.

All I ever had to do both with the lapband (6 months) and now for RNY (3 months) was see my doctor monthly for weight management, that what he would write in the chart, plus weight loss or gain and when I completed the requirements they were faxed over to the surgeons office

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.
