Ok you are you dr can go on OAC Obesity Action Coalitions web site and download the forms you Bariatric dr has to fill these out and fax it to Bariatric Advantage here is phone number on form for questions 800-898-6888 my dr faxed it to them and the next day I got a email they were be shipped out When you go on OAC web site there's a place for dr to download the form They give everything your dr list on form for 3 months about 3 weeks before you run out have your dr refax the form for your second 3 month supply This is only good for 6 months but it helps

I am pre op still jumping thru some of my insurance hoops. At last visit the director gave me samples of baratric advantage and of Celebrate. I really like the Celebrate site they have banana' date=' vanilla cake and other flavor Protein shakes as well as Vitamins to take before surgery. I ordered the Vitamins and a 10 pack of mixed Protein shake packets today. I am excited and nervous..[/quote']

You can go to the new life supplements website and its gives a comparison chart of all common bariatric supplements. Its pretty interesting to see how they stack up against each other.
