Does anyone know how to contact the doctor at the hospital in Grande Prairie that does fills?

Hi GP Bandsters, I'm getting my band on tues in mexico.

Dr. Jim does fills in GP hospital, but you need a GP doctor to fill out the paperwork to send you.
