myfatisnotaflaw 33 Posted January 21, 2015 Anyone did NOT have the 5 year weight history with their PCP? I'm new typo the area so I've only seen this PCP twice about weight concerns. I'm afraid this who'll be an issue????
SandeeD 363 Posted January 21, 2015 Do you have a yearly physical? If you do, your doctor should be documenting your weight and bmi. I freaked out because I totally forgot that my pcp kept track of my weight and bmi each time I see him. He actually had 7 years documented for me. What about your previous pcp, can you get your records?
SandeeD 363 Posted January 21, 2015 Catherine, I see that your surgeon is Dr. Boynton. He was my surgeon and he's awesome! Chris is amazing as well. I have nothing but good things to say about them! I have horizon NJ as well. I saw Diane Palmer through kennedy in Stratford. I had no out of pocket expense. It was covered 100%Diane T. Palmer, MSN, Apn856-566-6459Just call and tell them you need a psych clearance. Shes very nice. Yiu may have to call a few times to get answer. Started the whole process in Maryland last June, moved to NJ in October, three insurances later finally submitting but they've asked if i can get my old PCP to write. Deep breathe. We've come this far.

What about your previous pcp, can you get your records? I just had a baby in 2011 so I've been seeing my GYN. I think since having my baby I onlynsaw my PCP once or twice. Maybe I should get those records from my GYN. I was my heaviest then too.

Tazi 33 Posted April 2, 2015 (edited) Hello everyoneSince Horzion NJ Health doesn't. Cover psych clearance. Can anyone give me the name of a great psychologist to go to in NJ? I live in Lindenwold. I find a psychologist but she wants 250 just to do a consult and write up a letter. Edited April 2, 2015 by Tazi

susansworld76 1 Posted May 15, 2015 https://www.bariatricpsychevaluations.com/ Do you have to give 5 years of weight history? I didn't go to the doctor until recently. Thanks!
Tazi 33 Posted May 15, 2015 Yes you do, even if you have the history from different doctors to help you fill in the blanks
