Silkylady 3 Posted August 3, 2013 I have Basic and it was a smooth process to get approved. Three month diet I did with primary doctor, nutrition classess, cardio, psych eval and barium test and I only paid my 35 dollar copay each time I went to a specialist....then 150.00 for the surgery...it took 3 days to get approved. I was sleeved on July 19 and just got the insurance statement. The surgery and hospital stay was billed at $59,545 but since it was with a preferred provider the insurance company actually paid $19,921.00. Arts137 1,811 Posted August 3, 2013 Yes, no problems, but my BMI was over 50. At 35 do you have any co-morbidities? And you'll need to document (via your MD) weight loss attempts over the last year, and do all the preps (which takes about 6 months). Share this post Link to post Share on other sites
