I have BCBS of Hawaii && precert is required. I just got my approval this past Tuesday!! Call ur plan && best of all KNOW your plan!! Ask what's their policy && the criteria to see if u meet it. I'm happy that I work for my insurance ...I know the policy about bariatric surgery, my benefits && found out my copayment.

Get it in writing from your insurance carrier, you do not want to be on the hook for thousands of dollars after the fact.
