My insurance UHC has a few requirements (physician supervised attempts to lose weight, Psych eval etc) I have to complete before they approve my surgery. Oh really.. mine just says: Physician-supervised diet must be documented within the last 2 years prior to approval. But I do have doctor documents showing obesity from 2007... HoustonTexanFan 6 Posted July 23, 2013 Yes I also have to do 6 month diet, but I was told that u could use documentation from prior weight loss programs as long as they were done within last 5 years.
