JAB7955 0 Posted December 12, 2006 My surgeon provided me with a list of criteria that would be required by United Healthcare, and the list states that UHC will require two documented medically supervised diets, which you have failed to lose weight. One the the diets must be within the last 12 months. Has this been your experience in dealing with UHC to get approval for the lap band? Share this post Link to post Share on other sites
tonya66 13 Posted December 12, 2006 I also have UHC insurance, I didn't have to have that. When I filled out the paper work at the docs office, I had to list diets I have tried at and failed the last 5 years and that is it. I would suggest you call UHC yourself and ask if they cover Lap-Band (the CPT code is 43770), also make sure your employer does not have an exclusion for WLS. When I call UHC, they said the only thing that was requried was a BMI of 40. I was approved within 2 weeks. Share this post Link to post Share on other sites
