BUT you get a huge bottle of calcuim citrate chewables (500 i think).. Vitamins.. B12 sublingual.. and Iron. All thru Bariatric Advantage and bariatric approved. In the middle of the page, a small way down, you will see an icon (not a tab) for RECOVER PROGRAM. Click on this, and it will take you to the page that has the PDF form. This is not for samples, as best I can tell. It is for assistance with Vitamins for those that cannot pay, or it would be a financial burden. Its worth a try though!
