hip covered?

I'm covered by hip and I have no idea if they will cover a gastric sleeve surgery. Has anyone heard of hip covering it?

I have HIP, it's covered.

3 month supervised diet under bariatric surgeon or 6months supervised diet by PCP.

BMI of 40 + or 35 w/ comorbidity

Typically stuff... Look at their website, it's pretty straight forward.
