onmyway1978 2 Posted April 6, 2013 My insurance requires me to have 6 months of documented dieting which I go to my 5th month appointment next week and 5 years of medical history. I have done everything that was asked but I am having a really hard time with the 5 years of medical history due to my insurance changing every year for the last five years. I have been with the same company for 8 years now but to keep insurance cost down they have changed insurance each year. I do not even know how to start on getting 5 years of history or if I even have 5 years of history. Any suggestions? Share this post Link to post Share on other sites

Itsme! 98 Posted April 7, 2013 I don't have a primary care doc. I haven't seen my OB for 2+ years after my second baby (I know, shame on me!), but I sent insurance my hospital notes from baby 1 (2008) and baby 2 (2010) and foot surgery (2006), each showing my weight, along with a list of all the diets and crap i have tried over the years, and that was good enough for them.
