sissytx 4 Posted March 29, 2013 I had to have 4 appointments with the nut. My last one is 4/22 and then they will submit. The waiting is hard, when I started it was only 3 appointments. axd0424 33 Posted April 7, 2013 I have Cigna. The required the following: 3 nutrition visits once per month...had to be once a month..no more. No less. The required one psych eval, 1 cardiologist visit and an upper GI. Also my last 2years of weights from my primary and a note from my primary. My bmi when I started was 35. So I also needed another factor. So I did the sleep apneA test which I knew would b positive. They denied me at first button doctors office helped get it approved. Took a year from start to finish.
