I had to have a BMI over 40, a history of chronic morbid obesity, 3 months of medically supervised weight loss attempt with PCP, over the phone nutritionist consultation, psych eval, and letter of recommendation from PCP. I have one comorbidity and was approved on the first try in one week. When I saw the insurance coordinator calling me so soon, my heart sank thinking I was denied. Scheduled for April 9. VSG Tommy B 10 Posted March 24, 2013 I have Cigna and am having a band to sleeve revision. Some documents say I need the 3 months weight loss program with PCP, which I have done. Others just say the surgeon needs to show mechanical failure of the band, which he can. In reading that some of you have been required to see a NUT for 3 months too, I am getting nervous. My surgeon is submitting to Cigna for approval tomorrow. I can't have the surgery for another two months due to work, but my fingers are crossed for a quick answer so I can meet any additional requirements and get approved in time to have the surgery in late May or June as originally planned. I don't know if Cigna has different requirements with conversion surgeries, but your surgeon/surgical coordinator should know.
