Letter to insurance company

As part of my approval process I have to account for my weight history for five yrs and two of these yrs I was unemployed and uninsured. This leaves me in having to write a letter to my insurance company explaining the lapse of medical documentation which I wasn't insured.

Has anyone prepared or have a letter to share as an example in formatting this scenario? You're help would be greatly appreciated!

Hey I had to do this with my emloyer first before I had to do it with the insurance. I just documented the times I was on weight watchers, when I tried the Atkins diet, south beach diet, diet pills, exercise and nutritionist. I was pretty thorough telling how much weight I lost and how long I stayed on the diets and then how my weight raised again. Hope this helps.

My doc did my account of diet history for me, even though it was the first time seeing him. He just wrote down all the past diets I had been on and stated that they didn't work. I would just write down all of the diets you have tried and the periods of time you tried them. They just want to see that you have tried and failed at other ones....which obviously we all have...lol good luck!

      Two months out from hiatal hernia repair.  Surgeon said to expect a lot more flatulence...something about the 'air' no longer being able to 'burp' out so comes out the other end.  That is my experience but have no understanding of why that swallowed air cannot be 'burped'. ???

        As I understand it since your stomach is smaller and not completely resting against your diaphragm anymore you no longer have the ability to "push" burps out as well. Plus, since its smaller and we don't digest slower the trapped air moves a lot quicker out of the stomach so its no longer available to burp out. Hence the other option for removal.

        Me too girl!! Are you in the full liquid diet right now? It’s sooooo hard!

        Not yet. I was told I only have to do 24 hours of a liquid diet. But I have my pre-op tomorrow so I’m going to confirm if I need to do longer.

      Down 33 lbs and slightly stalled, but I'm gonna reevaluate and push through. I started back to work last week after 2 years of being disabled due to mental health as well as my weight. It's a great job and I'm just so happy to have this opportunity at a second chance at life. Hope everyone is having their best journey ❤️ Together, we got this!
