Michelle1977 27 Posted January 24, 2013 I have BCBS of Tx. It took a couple of weeks for my Dr office to even submit for approval (they send them in bundles of 15 to 20 requests). It took 2 weeks for the insurance to get it and send back the approval to my Dr. I got the call this morning that they finally have the approval. I called in the Ins Comp last week and they told me about the approval. I met all the requirements though.
soontobehealthyandthin 18 Posted January 24, 2013 I have bcbs of New Jersey and was just approved this morning with no issues. They seem to be pretty good. Just make sure you have everything together before it is submitted to avoid getting denied. Good luck. I'm now scheduled for feb 27!!!!

lovietobeme 45 Posted March 11, 2013 I am not sure what the weight loss clinic sent in but I had to get a physc. aval, visit a nut, get my primary to write a letter, a sleep study, and history of obesity. I think that might be it!

IF you have the benefit, the medical policy requires a psych eval & if your BMI is below 40, documentation of two recognized co-morbidities (like sleep apnea & diabetes for example). Your plan (as ALL BCBSTX plans) may differ according to what your employer chose to purchase so best to call customer service for details. Good luck!
