On my Quest to getting a gastric sleeve, sometimes I have felt lost and confuse with Sooooo much conflicting information. So I went digging and have found some documents that are plan specific to Anthem BCBS of CA. Remember each individual employer may add or eliminate requirements from these guidelines(also there are different requirements between PPO and H M O plans). So please call your Anthem carrier to find out if any changes have been made to these standard Anthem guidelines.
