Traci J. 207 Posted January 2, 2013 Don't you have a regular Dr. that you'd go see. They should have an intake form with your weight for ea. visit, which they can then figure out your bmi from. I had to call 2 different Dr's offices for my intake forms, and then faxed them to the surgeon's office. I was approved within 48 hrs. Share this post Link to post Share on other sites
