Briannasmom 7 Posted November 10, 2012 My doctor fixed one for me at the same time as my surgery but they took care of notifying the insurance company. I didn't have to do anything. You should check with the person in your doctors office that handles the insurance and see what their policy is. I just had to cover the extra to the doctor after the surgery was over.
