My insurance was approved but I'm wondering do I have to pay the rest right away or is there a payment plan. I have blue cross of California.

They're all different. They will usually tell you to talk to the billing department at the hospital to arrange for a payment plan if you need one. The surgeon's office should at least have the contact info you need to get you started.
