almostsleeved28 6 Posted June 19, 2012 So my surgeon's office just told me I have to give them a personal diet history. They were not really explaining what they need. Does anyone have any ideas on what they might need? or what they might be looking for??? This is like one of the last couple of things I need for my surgery. joyful noise 101 Posted June 19, 2012 do they want a copy of your weight loss - what you ate and how much you exercised? I had to do a listing of everything I ate for 3 days and 1 had to be a weekend day. Then there was a 5 page questionnaire - allergies, likes & dislikes, etc. that was all pre-Nut. Krysty 11 Posted June 19, 2012 My surgeon used my primary care physician's records to document my weight over a two year period along with a questionnaire they gave me listing all the different diets I had tried, the amount lost, and the amount regained. I'd suggest you call your surgeon's office and ask them if this is something you need to put together yourself, or if they're going to help you with it. almostsleeved28 6 Posted June 20, 2012 I have to call them back an find out exactly what they want. They really didn't give to much detail over the phone. I know they are waiting for my letters from my dr about my weight and the 6 months worth of check ins.
