beautifuldiva 22 Posted May 26, 2012 Yes, i used the bcbs fep and the process was suprisingly easy. You need to have 3 months of dietitian visits prior to submission, proof that you've been obese(hate that word) for the past two years. You can prove that simply with just regular doctor visist weigh in records. You need the pysh eval as well along with proof that you've tried other diets and failed. carolinagirlz 20 Posted May 26, 2012 I have bcbcs fed also. It was really easy for approval. I had gone to a weight loss clinic last year. That qualified for my 3mo supervised diet. I met with psych and submitted med. records for two year period. They only need proof from any dr that you weighed in with. (gyn, pcp, orthoped, etc) I was approved in 2days after submitting paper work. And they cover a lot!! Hope this helps Share this post Link to post Share on other sites

carolinagirlz 20 Posted June 1, 2012 I have the basic. The only prob I have run into is my psych eval. req. by my dr was out of network. So I had to pay 225.00 out of pocket However, after everything is done my total for surgery is 450.00. That's drs and hospital. Not to bad considering my health!! Having to pay nothing out of pocket would be great, but paying $450.00 is not bad at all. . I have my last nut visit this month and then it's off to the insurance company for approval. Not wanting to have surgery until the end of Aug which gives my doctor's office wiggle room to get the date I want, Aug 30th. Good luck to all of you.. carolinagirlz 20 Posted June 4, 2012 I paid 150.00 for dr to do surgery, 150 for each night at hospital(dr requires 2nt) they cover 100% of surgery itself and all meds. I have surgery June 25th.
