The one where they just decide if you're a candidate, discuss surgical types, health/weight loss history and rcvd a script for reflux tests. Does this consult normally cost $481??? rdoactv 218 Posted May 5, 2012 I have Anthem. But havent met my deductable yet, my employer pays 1st half of 2500, I pay second half. They paid the full amount out of that account, rather than the copay. Anthem paid nothing.
