Back~To~Amy 307 Posted April 2, 2012 One of the criteria for insurance covering me is: The individual must have actively participated in non-surgical methods of weight reduction; these efforts must be fully appraised by the physician requesting authorization for surgery. Anyone know what do they mean by these efforts must be fully appraised by the physician requesting authorization? Thanks!
kimmy*custis 276 Posted April 2, 2012 Basically the Doc needs to list all your weight loss efforts, Weight Watchers, diet pills etc. I had to write down every weight loss plan/pill I have ever tried, wasnt too hard to get a good list quickly. Just make a list and take in to your next visit. Good luck!
Back~To~Amy 307 Posted April 2, 2012 Weight Watchers is emailing me a list of the times I was on the monthly pass. Do you think I have to show proof of weight for each visit?
emily_0192010 168 Posted April 2, 2012 All I had to do was write out a list of past diets/exercise attempts and what my approximate weight was during each. Nothing official. I have Aetna if that helps...
