Like Catie said, every plan is different. Just to share my experience, I had Aetna PPO. I did not need a referral from anyone because I had and still have a PPO plan (does not require referrals for anything). I was sure to be familiar with their policy bulletins, talked with customer service to be sure I understood the bulletins. Also, bonus was I did not have to do a supervised diet for the approval (thank GOD!).
