diane_revision 0 Posted September 8, 2011 My insurance (UHC) requires revised mental health evaluation for revision from lap band (2008) to sleeve (to be 2011). It consists of one hour talk and two hours of tests on first appointment. Then one more hour of talk two weeks later at second appointment. Then about a week to write a letter to surgeon for approve/not approve. Seems excessive. What was your mental health evaluation like ? Share this post Link to post Share on other sites
To Be Thin 2011 80 Posted September 8, 2011 I have Blue Cross Blue Shield of Michigan... it was not covered, I had to pay $200.00 for a one hour visit and that was it. The doc talked with me, asked me to set some goals, etc.. within a week she sent her notes to my WL doc and I was all set. I did feel it was unnecessary, but can understand doctors are sued over everthing now a days so most likely this was to protect them some how... I would do it again in a heart beat. ab76307 22 Posted September 11, 2011 I also have UHC and for my intial consult my psyc Dr. turned me down. He said I had too many underlying issues!! Stupid! I just have to go to another psyc Dr and get on a schedule for visits with her on a weekly basis, after three visits she wrote my approval letter however part of that was I still had to see her a few times a month to help with the changes that I'll go through after surgery both mentally and physically.
