Filling out medical history form

What is the official / proper way to state this type of surgery on medical history forms? I am filling out some paper work for a cardiologist and want to state it properly:

'Vertical Sleeve Gastrectomy'

'Sleeve Gastrectomy'

'Gastrectomy Sleeve'

I don't get what the word 'vertical' has to do with it....

Would love to hear what other think...

"Vertical" because they made a long skinny stomach (cut it vertically) instead of a short stomach (cut it horizontally, which wouldn't have worked at all...)

Vertical Sleeve Gastrectomy

vertical is because of the way the cut was made and the direction that your new tummy takes now

I listed as partial gastrectomy; many medical ppl don't know what a sleeve is.
