Written info on VSG as appropriate revision post band complications

Dr. Jossart,

Is there any data or written opinions or policies or anything available on the following:

  • VSG as band revision procedure
  • band complication statistices, particularly after band slips or pouch dilation

I?ve been fighting my insurance company for coverage for over a year. They finally took the band out but refused to cover a revision. They?ve thrown everything at me from the investigational claim (which I used your studies to refute) to too low a BMI. Now they?re saying that there?s no proof that a VSG is an appropriate revision surgery and have asked me for more documentation to show that VSG is used and is appropriate for band revisions. The only people in the world I can think of who might have information are you and Dr. Cirangle.

Can you help? Is there any article or statistics you could point me to? Some sort of letter or medical standards or even your medical opinion I could send them for the next round of review? I'm about to be unemployed and getting reimbursed for my surgery would make a big difference for my family.

go to pubmed.com and search the terms. You can use Dr. Himpens and/or Dr. Rosenthal as I believe they have both published articles on about 20 patients that were band to sleeve. Interestingly, one of the more common operations now is band to sleeve. More publications on this will be coming out in the next two years.

Thanks Dr. Jossart! Dr. Himpens has a new study he published last month. I emailed him and he's going to send me a copy. Take that Office of Personnel Management! I'm not giving up until they make sure they have a clear policy in place so nobody has to go through a year of misery and insurance appeals like I did.

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.

      Woohoo! I have 7 more days till surgery, So far I am already down a total of 20lbs since I started this journey.
