Is there anyone who has been approved by this insurance for lap band? I am interested in knowing how long it took and what was required for approval...

I can't answer to this directly as I don't have this insurance, but I always advise to call your insurance in terms of what is covered. It's best to hear it "from the horses mouth" and avoid any surprises later on.
