If its in the best interest of your primary doctor what can cause you not to be approved for surgery if you are well willing and able to stick to the guidlines and want to make this change and have diabetes due to weight?

1. It has to be included on your actual insurance plan.

2. You have to meet to weight requirements- usually 35-39.9 BMI with two comorbidities or 40+ BMI with no comorbidities.

3. Some plans require a supervised 3 or 6 month diet. Others require 2+ years of documented weight history above the specified BMI and some require a list of all diets (including dates tried, weight lost) you've ever tried.

I would personally call the customer service department and ask what the policy states.

Definitely call your insurance to get the details from them as every plan is a bit different and the plans change too. My plan last year was fairly strict and now all of a sudden this year they dropped a lot of the requirements they used to have so my approval went fairly quickly and smoothly. Good luck.
