Dr . Jayasleen in Dallas takes medicaid .. his website is www.texaslapbandcenter.com I believe .. His phone number is 972-566-2263 ... He is awesome !

Dr. Maese's office also takes Medicaid. His # is 214-234-0330.

His office is in Dallas, and he also has one in Ennis Texas. Hopes this helps.
