Hello Banders, I am 50 y/o 260 lbs with a BMI of 41.9. I have been thinking about the lapband for at least a year. I've been overweight for 20+ years. I was told that Cigna PPO can be very challenging to get an authorization even though they do cover bariatric surgery. I have been seeing a shrink for 8 years due to Major Depression. I was told a letter from him to Cigna would be fine but I need to know what it is that he is supposed to write to them? Anyone know? Thanks so much. I'm "so" happy to have found this group.

I would suggest asking the question to your insurance company. You may not even need it.

I wasn't required to submit letters, but I did get a copy of my PCP's dictation from our last meeting and submit it -- no formal letter though.
