courtdemers 0 Posted March 1, 2012 My husky insurance Said they DO cover the lap band if I get a doctors referral.. What type if things should I say to my doctor to have him write me one? honk 780 Posted March 1, 2012 Also ask your insurance what their requirements are. Often you need to have a Body Mass Index (BMI) of 40 or more; or less if you have co mobidities (high blood pressure, high cholesterol, ect). Some insurance also requires you do presurgery diet for a period of time before they will approve the surgery. I had Blue Cross and my policy did not require any waiting period. NWgirl 574 Posted March 1, 2012 I am 23 and my insurance didn't specifically say they needed a referral, but I ended up calling my pediatric Dr (birth-20) to get weight records and he decided to write me a letter stating that I had tried formal weight loss programs to no avail and that I would be a good candidate for the surgery. All that helps your lapband dr too so he can submit to insurance as well, any diet programs you have tried, followed by dr. or not, any other dr. too who can provide weights.
