JerseyGirl80 12 Posted October 21, 2011 My surgeons office got proof of me being overweight themselves by requesting my medical records from my primary doctor. They will probably ask you for your primary doctor's information to contact that office for your records. I didn't do any running around to get them any information, they did everything themselves. JERSEYGIRL12111 2 Posted October 21, 2011 My surgeon only needed what diets I have tried and failed. I have Blue cross/Blue Shield of NJ. I know each insurance company is different. So I woulld at least give them a list of diets you have done and then take it from there.
