Any advice for my Primary Care Doctor?

Hi All, I in the process of finishing my tests for lap band. All i have left to do is the Cardiac clearance, Psy eval, Pulmonary test, and Nutrition class. I will be finished my the second week in Feb. My insurance (Blue Cross PERSONAL CHOICE of PA) requires this:

The individual must participate in one of the following:

  • <LI type=circle>Physician-supervised nutrition and exercise program (eg, primary physician, Optifast, Medifast) over three months consecutively prior to surgery, which includes the following:
    • <LI type=square>Dietician consultation <LI type=square>Low calorie diet <LI type=square>Increased physical activity
    • Behavioral modification

      OR


    <LI type=circle>Multidisciplinary surgical preparatory regimen (ie, facility-based, pre-surgical program) designed to improve surgical outcomes, reduce the potential for surgical complications, and establish the ability to comply with postoperative medical care and dietary restrictions, which includes the following:

    • <LI type=square>Reduced-calorie diet
    • Exercise program in order to improve pulmonary status
    • Behavioral modification program

Ok so I set up a appt for the next 3 months with my primary doctor. What will he need to do/or not to do? Does he just record my weight, take notes, have me start to diet. He is 100% for me having this but I dont want his notes to ruin my chances of getting approval. His written notes are what is going to be sent to the insurance company. Can anyone give me guidance on what to DO or NOT TO DO when I see him? so far with my tests results I have a 44 BMI, High blood pressure, GERD, Osteoarthritis, lower back problems and mild sleep Apena. Yep! He should note your willingness/ability to comply with the diet, and any exercise program you've undertaken. That's pretty much it! The idea is to show that you're able to be dedicated to the effort, and his notes will just support that. Tell him what you're doing, he'll take notes and measure your vitals, and after three months you'll have what you need.

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.

      Woohoo! I have 7 more days till surgery, So far I am already down a total of 20lbs since I started this journey.
