Does United Health/Oxford Liberty Plan pay for the Lapband

I currently have GHI through my husband and I am signing up for United Healthcare/Oxford Liberty thru my job. I already know that GHI wants you to go through 6 mos medically supervised diet. Does anyone know what the requirements are for United Health/Oxford from what i'm reading it seems like a letter of medical neccessity and a BMI above 40. If anyone can shed some light on this please let me know I would appreciate it. Thanks

Hi, I have United Health Care/Texas. My paperwork was submitted to them on January 31st and I was approved on February 15th (My surgery is on March 22nd). I didn't really have any severe comorbidities (although I have sleep apnea and PCOS). They typically just want you to have a BMI of 40. Truly, depends on what your employer has outlined in their contract with UHC.

Thanks so much for your reply. I hope my process goes as smoothly as yours did. From what I have been reading yes that is true a BMI of at least 40 and comorbities. I have a BMI of at least 60 and Comorbities of sleep apnea and high blood pressure. Good luck with your surgery and a fast recovery. Keep in touch let me know how things go.

I have united health care in missouri and I was banded in dec. And they payed for it!

      The 14th was my day. I am home and recovery is going pretty smooth. They even let me walk out of the hospital. Picture of me in recovery curtesy of my boyfriend lol.
