You can look up samples on google. I had to have one of these too. Basically its just your dr explaining to the insurance why he thinks you should have this surgery. Any medical problems, how long he has been treating you, different weight loss attempts. I have a bmi of 44 also, I'm sure your dr. would be on board. If not, do you have another dr you could go to to get it?

I can tell you this from experience.... Aetna insurance sucks! they paid all but 5,000 of placing the ban because it went over Aetnas LIFETIME bariatric cap of 10,000. 6 months later I had to have my band emergency removed and they refuse to pay anything else. therefore I now have a bill of 16,000 that SAetna abolutely refuse to pay so I'm screwed!!!!

My surgeon's office had a pre typed letter and my pcp just had to fill in the blanks. You might want to ask your surgeon if he/she has anything similar.

Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
