Medical Necessity and Medical Clearance Forms

Recommended Posts

I talked to my insurance coordinator yesterday and she said after I do my Psych visit and my nutritional counseling all I still need to have is my Letter of Medical Necessity and Letter of Medical Clearance from my PCP. I went to them and got a referral so I thought that took care of all that but I guess not. I called and left my dr a message about this and one of his nurses called me back and said she would "get it taken care of". I'm guessing they send that over to the dr? Any advice on this?

Be persistent and keep a copy of the letter that he sends. I have a great PCP but sometimes he is a little slow:)...A friendly status chack goes a long way..

You need them to actually write a letter saying you are clear to have this surgery and it's neccessary. My PCP had me write up all of my weight loss attempts, how much I lost/gained and what failed in the attempt (most dr's don't make you do that, but mine wanted all of it for my chart). Then she wrote a wonderful letter stating I needed this or I could get HBP, diabetes or have a stroke. She talked about my weight loss attempts in the letter. I just had my husband pick it up for me and I took it to my surgeon's office during my next visit. Be sure to make a copy because my surgeon's office thought they lost mine on my last visit, but they found it right away in my other file.

Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.
