My surgeon is listed in the wrong state

In the surgeon section of the forum, he is listed in Connecticut but he is really in NY. When you go to the top of the page and click Surgeons he is there in the right spot, but not on the forum. His forum is blank and I think it's because he is in the wrong spot. How can this be fixed?

And sorry if this is the wrong place to put this but I couldn't find anywhere to report problems.
