    Gerd with weight loss Plateau

    Sharing my experience as a VSG patient who had terrible GERD prior to surgery and a hiatal hernia - I was taking 20 mg of pantoprazole daily and STILL felt painful heartburn. I had a hiatal hernia repair (and gallbladder removal) at the same time as VSG. In the first 3 months after surgery, GERD was still pretty bad (which can be normal for most bariatric patients). My doctor had me on a proton pump inhibitor (Omeprazole) for the first few months. I had been on this type of med for 3 years prior to surgery, and this is the type of medicine you can't just quit taking cold turkey without some seriously bad flare ups to follow, regardless of having bariatric surgery or not. So after 2-3 months I started to reduce the frequency of taking the medicine gradually. After a month or so of reducing, I switched from proton pump inhibitor to an H2 blocker (over-the-counter famotidine) and only took it on an as needed basis. At first I still needed it often, but now I am 13 months post-op, have lost 80% of my excess weight and can't even remember the last time I needed to take any anti-acid medication. I have almost zero issue with GERD anymore. I attribute that to the hiatal hernia repair + loss of visceral fat (creating pressure on the stomach) + gradually reducing reliance on medications instead of quitting cold turkey + knowing and generally avoiding trigger foods. Hope that helps. Just wanted to share a story that is counter to the common assumption that VSG and GERD always lead to bad post-op experiences. I lost weight at a very consistent rate, but this can be different for everyone. Sounds like you are getting support from your dietician on this, which is a great resource. Good luck!     Lots of nerves are cut during the surgery so messages either don’t get through or feel different until you’re fully healed (about 8 weeks). It’s why it’s important to stick to our plans, portion size recommendations, to eat/drink slowly & listen to our bodies carefully. When they do return you’ll be more conscious of them & may find they are different to what you knew. For example, you’ll discover your signals for real hunger vs head hunger are different (no cravings for a specific food, flavour or texture, & I feel restless) with real hunger). Some people sneeze or their nose runs when they’re close to feeling full or had enough. The tightness across your upper chest (makes me want to thump my chest to alleviate it - it doesn’t) is your restriction. This usually doesn’t kick in until you’re on solid food & your nerves are more fully healed. Yours may have started early. It’s usually your signal you’ve eaten too much &/or too quickly. Remember your tummy is quite small now, it doesn’t extend as low down in your abdomen & it doesn’t take much to fill it. Sometimes certain foods can set it off too if they sit more heavily in your tummy. It’s considered a benefit of the surgery. It can fade in intensity over time (years) & we also get better at understanding how much we can eat & so don’t feel it as often which really is the goal. All the best.

    December Surgery Buddies!

    Decided to have surgery because my normal glucose went from low 80s up to mid 200s and a few low 300s. Tried metformin for a couple years didn't help then Ozempic which messed up my gallbladder and pancreas. Had my surgery on Dec 27 (198lb/245gl), was released from hospital on Dec 29. Went back into hospital on Jan 1. My intestine got twisted then caused blockage and my stomach was not closing of stomach acid and it was free flowing for 3 days at home. The must painful experience ever. Released from hospital on 7th. Basically started was like my first day out of hospital from surgery. Had my 2 week post-op yesterday 9th moved to soft foods and can do light strength training... Super excited. (184lb/83gl)

    Where are you on your plan? Are you still doing the protein first thing. What does your eating look like? For you, what is dumping? What specific symptoms? What foods cause it? Are you able to connect your other symptoms (nausea, dehydration, diarrhea) with specific foods, drinks or other activities? On the dehydration, is this in spite of getting appropriate levels of fluids? What levels are those for you? Have your doctors done blood work? Was there anything of note? I've had on and off anemia and nausea that I can connect to specific mental issues and resultant behaviors which even though I am aware of the causes they still sometimes get away from me. For me, I Just have to push to do what I need to regardless of how much I don't want to when I'm going through a funk. I'm a big 'ol dumper on sugars and fats but fortunately I don't have much issue keeping those in check. I know for some people fats and sugars are go-to comfort foods that are tough to get control over, though not so much for me. I've reached out to therapy on a couple of occasions when I simply could not figure out the funk, which helped dramatically. Then the advice and coping skills I learned are always within reach. This is a weird thing to bring up, but I am curious if anyone else has experienced this: I had a gastric bypass back in February and for the past 2 or 3 months it seems like I have been having large and frequent bowel movements. I often go 2 times a day and sometimes even 3. It feels like I am going more now than I did before my surgery even though I am eating less than half of what I used to. I'm guessing this is due to the bypass, but it just seems like there is too much coming out of me for the amount of food going into me, it doesn't make sense.

    Girl you and me both my hunger isn’t really there more than my thoughts of wanting to eat food to feel normal again. Its been emotional for me and bc of my sampling food it makes me feel more down. It’s frustrating not being able to drink water or pretty much anything my water/ fluid intake is so low. Im soooo over these protein shakes i bought and the thought of them now has me wanting to gag lol. I’m not reallt taking in any protein shakes and just doing soups but can only do 2/3 ounces at a time. I can do yogurt i had half a cup for lunch and a few tablespoons of tomato soup for dinner. Feel free to message me anytime my surgery date was 12/18 im happy i got the surgery but definitely want to feel normal again. I’m being patient but if i could have at least be able to drink water it would be easier. i had a hiatal hernia and its been so hard to drink is anyone else feeling that feeling? Then when i do drink its like im swallowing air 😫😩😫

    I’m allowed food, low carbs though. Eggs, yoghurt, cheese, Quorn sausages, green veg, Ryvita, some fruit. At least 2l of water per day, 7-14 days pre surgery

    Hey i was sleeved on dec 1, first month down and yes my mental is all over the place at times i regret doing the surgery still questioning myself even after a month. Im still on soft foods i can eat mashed potatoes and pasta but i feel guilty because of the carbs after i eat it 😅 but it taste good. And yes i had a qeak moment Saturday i had a cookie from panara bread only ate a small piece but my belly was fine. But the struggle is real you qill over come

    Restriction

    300-500 calories a day at 6 months seems low, at least by my bariatric teams outline and booklet/plan given to me. I'm at 4 months and eating regular food at 800 per day. I'm told its right on track. Have you spoken to your doctor/dietician? Perhaps do as I was told instead of three meals a day - have 5 small meals to get you to where you need to be? Either way, I'd check with your doctor.

    I bought protein shake in powdered form & bought yoghurt drinks when I discovered them. (Be careful some have a lot of sugar/sweeteners & are actually low protein.) I didn’t realise I could make them myself so easily. LOL. I bought some supposedly flavourless pea protein to make my own smoothies but it was awful. All I could taste was peas (& I love peas but pea protein powder blah!). Some good tips I use when eating with others is not to eat while others are speaking. It slows you right down. And it means you give your full attention to others. Plus I put my cutlery down & lean back in my chair between bites even when eating alone. My last tip is to cut your food into small bite size pieces before sitting at the table. Of course you can’t do this when you’re out. I still do this at home. It’s like I’m prepping a meal for a toddler. lol! Use a small plate like a side plate for your meals (still do) to keep your portions small. I also used small ramekins. And I used a teaspoon initially & then a cocktail fork or spork instead of full sized adult cutlery to keep bites small. Some use children’s cutlery.
  11. TW: Mention of ED I’m about 4 weeks out from having this gastric sleeve surgery. Right now I am on the soft foods stage. I have one week of it left. When I was on the liquid diet I was craving food watching food videos and just could not wait to eat again. Now suddenly I am disgusted by all foods, and I do mean all foods even my “favorites.” I look in my fridge and most days I’m just like “I would rather not” and I have to force myself to eat but normally halfway through I’m like I can’t do this anymore. Ive also been experiencing a “twisting” feeling in my stomach when eating or drinking which is very discouraging trying to get the food down. I’ve even been doing this thing where I’ll chew something up only to spit it out. I struggled with binge eating disorder pre surgery and got treatment for it & what is making me nervous is that I heard a lot of people with anorexia/bulimia explaining their eating disorders and some of the things I’m experiencing are red flags for anorexia and I’m just afraid of going from one spectrum of eating disorders to the next. Did anyone else have this same repulsion with food? I am currently seeing a therapist and am planning to tell her all of this so I can get the help that I need. Sorry this isn’t so positive but I am just wondering if anyone else experienced this and if they had any advice or if it got better? Thank you!!
  12. I was barely eating 900 calories at 6 months (almost a cup of food for a meal depending on what it was & one snack) & at my goal weight. But my stats are different from yours. I’m likely older, definitely shorter & I wasn’t & still aren’t overly active. (I eat about 1600 now to maintain my weight.) You have every right to ask for greater guidance around calories, portion sizes, macros, etc. from your nutritionalist if it makes you feel more confident about your choices & comfortable about what you’re doing. If they refuse you may have to find another one who is willing to support you. Yes, I know calorie counting is considered out, but for us it is an important consideration especially until we feel more secure in what we’re doing & helps us to lose/maintain & maintain our health. We need that guidance now as most of us didn’t know what the hell we were doing in regard to appropriate foods to eat, nutrients our bodies needed, appropriate portion sizes, etc. We’re new borns about a lot of this in the beginning. If we knew it all & could do it we wouldn’t have needed the surgery. Honestly, I wasn’t given calories goals or specific macro except protein 60g for me. Just portion size, and low carb, low fat, low sugar. I was okay with that though I would still randomly check for my own interest. I did a lot of my own reading, and was very careful with what I chose to eat & checked with my dietician at every fortnightly appointment. As I said, you have to feel comfortable & confident about what you’re doing. After those first heady weeks of large weekly weight drops &, regardless of our starting weight, our rate of loss slows. One to two pounds a weeks is okay especially as you are on the homeward path now. Remember that 1-2lbs a week is generally considered a healthy rate of lose for anyone losing weight. Wait until you’re almost there & you’re losing ounces not pounds a week. Believe me, the last few pounds to lose are a b*tch to lose.

    I’m in my second week of puréed diet. I also had two weeks puréed. My VSG was 12/04 and I’m looking forward to moving to soft foods next week. During the puréed, I have eaten ricotta bake; mashed potatoes; grits and eggs. Otherwise, protein shakes….

    NO TRACKING ?

    I wasn’t required to track. I sort of did initially on random days for own curiosity. I did keep to the portion sizes I was given. I still do random checks to keep me on track & ensure I haven’t let things slide especially with new foods or recipes. I check protein, sugars, calories but don’t keep a formal record. I keep a mental tally of my daily protein intake but only because I have a protein malabsorption issue. It works for me & that is the key: finding out what works for you. If that’s tracking great. If it’s random tracking/monitoring or it’s none at all that’s great too. As long as it allows you to maintain, is manageable & not consuming you, all is good. I put on a good 2kgs/5lbs at the two year mark but didn’t know why. We worked out I wasn’t absorbing my HRT after my gall removal (why I don’t absorb protein too). Was put on a HRT patch & I slowly lost half of it over about 6 months again without changing what I ate or my activity. I’ve pretty much lost another kilo and now, nearing my 5 year mark, am pretty much what I weighed at my initial stabilised weight depending on the day. I should add I am very careful with what I eat & how much I eat. Still eat slowly. Still take small bites. Still eat to a routine most of the time around what & when I eat (I’m a clock watcher for my meals & snacks). Still very conscious of whether I am really hungry or just head hungry. Still very conscious of eating what I need not just how much or what I want to eat.

    December Surgery Buddies!

    PROGRESS UPDATE I had my gastric sleeve surgery on December 18th, 2023. I was nervous going into the procedure as I had never had any surgical procedures before, but it went relatively smooth. I was in a lot of pain while in recovery, to the point where the team had to push oxy and fentanyl simultaneously to help with the pain. Once the team was able to help me manage the pain I was in pretty good shape. It has certainly been an adjustment but it wasn't as daunting as I was expecting it to be. Its been about 2.5 months since the surgery and I've lost almost 60 pounds; I weighed in on at the hospital at 366 and yesterday weighed in at 308. My highest weight was 475lbs so all-in-all I've lost 167lbs utilizing diet, exercise and the sleeve. I found it fairly easy to comply with the post-op diet and have moved on to solid foods. I'm not hungry often but I make sure to eat at mealtimes to continue meeting my goals. I consume, on average, between 80g and 100g of protein a day and consume less than 900 calories a day. I don't consume very many carbs as a) they tend to contain more sugars and b) they tend to make me not feel very good. I do still have a bit of a sweet tooth but that has been easy to manage with the occasional chocolate chip. I've also completely kicked sugar and have switched to monk fruit sweetener with erythritol. I also dabble with keto recipes, mainly deserts as I find that some "entree" recipes contain way too much fat for my liking. My family, friends, and coworkers notice the weight loss more than I do. I'll see it a little bit here and there but not to the extent that others do. I still see myself as that 475lb guy that struggles with his weight and his health. I know that it isn't true, but that is still how I view myself. My clothes are fitting me much looser and I think I see some weight loss in my face and neck. I still have a lot of excess fat and tissue around my midsection and especially the "love handle" region. I definitely suffer from body dysmorphia as I only ever see the overweight me. My overall goal weight is 250lbs, which is only 58 pounds away which sounds crazy. That said, I would throw a party if I reach 225lbs as that is my "best case scenario" goal. I genuinely believe I will achieve both goals but there's always that part of me that doubts it. This has been the best decision I've ever made for my health and I wish I had made this choice earlier.

    Omg, i felt the same about them not doing it because i too didnt feel that full feeling or discomfort when i ate or drank im on week 3 post op puree/ soft diet 😅. The gas was real alot of belching my surgeon said that was normal. And yes it does get better i noticed yesterday that the gas qas not as bad. But im down about 15 lbs since the surgery i weigh every week on the same day so it helps . I told myself im not going to weigh agian til the end of the month just so i wont go bat **** crazy. You got this 👍. The process is real the journey is rewarding in the end

    As I stated I’m not in this for weight loss. My issues are belly fat and working hard to get rid of it since I couldn’t workout for 15 yrs because of regurgitation, vomiting and pain with my hiatal hernia. My question was only asking about GERD patients losing weight slower than regular obese patients. I don’t have any food issues and my dietitian has no worries about me. I’ve always carried good muscles in my skinny legs. I don’t compare myself to no one I just can’t believe these bariatric weight loss videos people have actually lost that much weight in 6 months as they claimed especially how huge they were. My reflux has always been horrible and I’ve been on medication for 15 years daily. Now I can only stay off for 4 days but I’ll have bad silent reflux. My GERD is unmanageable even with a strict diet. My surgeon thought since I never had GERD with my 2 older kids maybe it would subside now. This all went downhill after I had my 3rd child.
  19. Hello all, We are back on...Day 4 of liquid diet. 11 days before the bypass operation. My cravings (pizza especially) come and go fast. I have been dreaming of food. On that note i have been getting a little repurcussions from my family as its going to happen very soon. Some comments -You wont be fun anymore. Like you wont drink and eat with us! - There is no point to order Pizza with you as it was my favourite meal and I can only eat 1 slice and other comments... Its making me feel bad and questionning but I am doing this for me and not others! I need to stay strong!

  21. I'm so sorry you are experiencing this!! This isn't a you problem!! If you aren't losing sufficient weight you are either being given poor advice from your nutrition team, poor medical support from your surgeon, or it is possible your body is under significant stress for whatever reason and isn't losing weight. There are rare cases when someone with a sleeve doesn't respond but often those are people who have low starting weights. If they had a high starting weight and don't respond to the sleeve surgery, these people generally end up revised to bypass or a DS/SADI, and then lose weight, but again, that should be a convo being initiated by your surgeon as a future possibility if food modification and medications don't work. Some people take GLP-1 meds to help jumpstart their weight loss if the surgery hasn't triggered it, but again, at the 6 month mark this should be something the surgeon initiates conversation about. You shouldn't be living in fear of your appointments with them. If you aren't feeling supported you might consider getting a second opinion from another bariatric surgeon in the area not affiliated with this practice. I strongly believe in second opinions when talking things like surgery... It may sound like I'm being harsh on your team, but lets be clear. They made a nice chunk of money off of your surgery. You deserve good aftercare!! Some surgeons, like my own, believe that the best way to lose weight in the first 6 months to a year after surgery is through being in ketosis. This involves a good protein intake (60-80 grams with the sleeve) and carbs below 50 total carbs or 30 grams net carbs. You can get pee strips to test if you are in ketosis. Once in ketosis you should go through regular periods when you lose some weight, followed by periods where you lose none as your body stabilizes from the previous loss and recalibrates. If this doesn't happen, I'd definitely be communicating with the surgeon about it! What dietary advice did your team give you? Hydration is important for weight loss. If you aren't able to eat enough calories or drink enough water your body will go into starvation mode like @summerseeker mentioned. This is a huge stressor to the body!! And huge stress will cause weight loss to stop. Sleep is also crucial to weight loss, often more important than exercise. If you aren't getting regular sleep for enough hours per night, this can stall your weight loss. Physical activity of some kind is important, but it accounts for less weight loss than proper nutrition and sleep. And if you are under calories and under hydrated exercise will just further stress out your body.

    That’s sounds more understandable I went into the office today and to my surprise I was 228 he said I misunderstood the diet plan lol I was not to start soft foods til two week after the surgery not on week two which makes me feel so much better because it’s easy to drink but wasn’t as easy to eat those puréed foods. my incisions are fine the biggest one I’m assuming is where they took the stomach out is bumpy also lol 😂 I feel fine to get up and walk about but after a certain point it’s over doing it this week I’m sick of being home so I’m heading to the mall today to exchange shoes for my kids the walking should be okay. Doc says walking only til week 6 post op. Which is a bummer bc I have a workout bike I set up last night too excited to use lol

    I follow all WLS guidelines so what does any of that really matter. I am allowed to eat some carbs as it’s limited and I don’t weigh my food. I use a small plate and I know what my measurements are. I do drink 30 minutes before and after. If I want sugar I’ll eat it within my own guidelines since I’m working out and that’s only once a month cravings. I’m only 5’3 and carry my fat in my belly. My surgeon has no problem with my stall since I workout 4-5x a week. I’m more worried about the daily medications I take for GERD for the past 13 yrs which isn’t good. They have me on 2 acid reflux pills and antibiotics so idk if that’s an issue too. I can lose weight I’ve always been thin all my life except after having my last baby which caused my hernia and issues; by then I couldn’t exercise no more gained weight plus perimenopause belly weight gain on top of that has been hard. The first pic on top was 1 yr before surgery the second pic is recent on new years.

    Mmmm Cheese. Good. I ate low fat Jarlsberg from solid foods but I did revert back to full fat after a few weeks cause it tasted nicer. And would sprinkle Parmesan on my bolognese & in my omelettes. Close to goal I would eat Brie, Camembert, a soft blue if out socialising where there were snacks but not a meal, A couple of pieces were ample. Full fat is so much yummier but it’s all about how much you eat each time & what else you’re eating. Saw something recently about there being something in cheese that makes it addictive. I can’t recall what because I don’t care - it’s an addiction I’m willing to indulge.

    1.5 years post op weight regain

    I highly recommend looking at the videos on the Pound of Cure channel on You Tube, which go into detail about how to eat to lower your body's metabolic setpoint. Foods that are ultra-processed and high carb push your body's setpoint, the weight it wants to maintain, higher, where nutrient dense foods like leafy green vegetables, beans, nuts, and seeds, make your body want to maintain a lower weight. As I have been preparing for surgery, I found the Pound of Cure book on Amazon and implemented many of the basic changes it recommends, such as cutting out processed foods, breads, sugar, and artificial sweeteners, and eating more vegetables, fruits, and beans. I lost 15 pounds in around 3 months without counting calories or ever feeling hungry. And I can also say that in the past several weeks, as the holiday treats have infiltrated my house, I have not been as good about eating that way, and to no one's surprise, my weight has shifted upward. I will be returning to better nutrition as soon as the last of the ham, sweet potatoes, and pie from Christmas dinner is out of the house! I recommend starting with these videos that I've linked below (and then, if you're like me, binge watching the rest of Dr. Weiner's channel). I find his advice to be a common sense approach to nutrition that you should be able to implement long-term without a lot of pain. Hope this helps!
