    Deciding between bypass & sleeve

    I have always had a dilemma about the sleeve, because patients can expand their stomach pouch, by poor compliance. The majority of people who have bariatric surgery experience psychological problems with their relationship with food rather than a physiological problem. Therefore, not withstanding a brain transplant, I don't think a sleeve would be successful for such individuals. I think that's one of the main reasons for revision from sleeve to bypass. With bypass, this is my own experience, you can eat as much as you want, but you don't put on the weight. I feel sleeve is suited to people who need a helping hand to lose weight but are strong enough to comply with a lifestyle diet change. Those of us who would continue to struggle with our relationship with food regardless, I think are better candidates for the bypass. The bypass comes in various strengths. There are several variations of the bariatric gastric bypass surgery, including: 1. Roux-en-Y gastric bypass (RYGB) 2. Mini gastric bypass (MGB) 3. Biliopancreatic diversion with duodenal switch (BPD/DS) and While Roux-en-Y gastric bypass, mini gastric bypass, and biliopancreatic diversion with duodenal switch are the most well-known variations, there are other less common types of gastric bypass surgeries, including: 1. Single Anastomosis Gastric Bypass (SAGB) 2. Loop Gastric Bypass 3. Sleeve Gastrectomy with Transit Bipartition

For comparison, because I know your other option is a revision to bypass, I am 5 months post RNY and I can already easily eat a cup and a half of some foods, such as soups, salads, or yogurt and berries. I can often eat 3-4 oz of meat, or a full can of tuna, plus veggies along with it. My brother, who is about 15 years post-sleeve, can finish a regular plate of food in a sitting (by which I mean a reasonable serving of something like chicken, veg, and starch, not what they give you in a restaurant that has 1800 calories and is enough for 3 people). SADI will probably change your metabolism in a more powerful way than bypass because that is what the research shows it does. But since you've already had a sleeve, it's possible that neither revision will have the full effect on you that it would someone who is getting a surgery for the first time. And whichever option you choose, I do think in the long term, it's less about whether you can eat a certain volume of food and more about what food you choose to eat. Basically all bariatric surgeries typically result in having more capacity the further out you get, so it's what you do with that capacity that makes the difference in success over time. I can tell you that my brother has regained about half the weight he initially lost not because of how much food he can eat but because of how much beer he drinks every day (he admits as much). And if I regain weight in the future, I already know it will not be because I am eating 6 oz of chicken and 2 cups of steamed broccoli in a sitting. It will be because I haven't managed to curb my sweets cravings (and yes, I can still eat plenty of sugar and fat without getting sick) and too often give into the convenience of processed foods and simple carbs. Those are my weaknesses, so that's what I'm trying to work on now while the effects of the surgery are still fresh. But no surgery can fix it for me, unfortunately. Basically, any revision you choose at this stage will give you a new and more powerful tool to work with. No surgery will address the underlying destructive habits that lead to weight regain.

    Hello. What stage are you on? How is it going?! I am 13 days out. Post SADI revision surgery. My recovery has been a little more challenging that it was when I did my sleeve.
  5. I had a revision for GERD. began regaining weight at 6 months post revision. I've had issues with rapid gastric emptying after my bypass, I feel limited restriction and have blood sugar issues. Most people only get these issues with sugars or simple carbs ie: dumping. I get it after everything. I started wegovy a week ago and it seems to be helping.

    Sorry for the semi-duplicate post. I was falling asleep while I was writing it, and some of it disappeared. Good luck with your GP, surgeon and dermatologist. I have also dealt with hives, even before my original gastric bypass surgery. We couldn’t find any causal reason. After trying several medications alone, my doctor and I decided to try a “cocktail.” I was on 4 medications, but I only recall Ranitidine and Zyrtec. It worked! I had the gastric bypass a few years later, and the hives left me alone.$!
  7. Pro Care Health multi with 45mg iron is what I took after my sleeve and my levels were always good too. Now I had SADI revision so I take their SADI specific one with a little more iron. I haven’t done any labs yet but I feel fantastic so I’m expecting them to be good.

  12. So sorry about all you’ve been through. I haven’t had a revision so I can’t offer a suggestion about that or share my experiences, but I wonder if your cough could be a symptom of reflux (GERD) especially as the asthma meds don’t work. Has anyone suggested it? If not it may be worth further investigation by your doctors. If it is GERD, a bypass (mini or full) would be the way to go.
  13. Hi there! Been on my surgery journey for a few years now and I'm finally coming up on my RYGB surgery date in a couple weeks. It might just be nerves but over the last couple of months I've started to rethink whether I'm making the wrong decision with the going with the bypass. I spoke to my surgeon about a week ago & expressed I might want to switch to SADI. She said it's ultimately my choice, but she feels like RYGB is a better fit for me personally as I have a history of GERD (currently mild) and that she's concerned I might lose too much weight with SADI - I need to lose 100lbs. I've been reading & watching a lot of posts from people that have had each procedure & I'm really concerned with the idea of throwing up & having to eat a couple of bites per meal the rest of my life. The thing is, I rarely see anyone post complications with SADI. They seem to eat small meals but not as small as restrictive as RYGB patients & while they experience diarrhea - I don't see posts about vomiting. I mean, does everyone experience vomiting & dumping syndrome with RYGB? Does anyone know people that have had poor experiences with SADI? Ultimately, am I making the wrong decision by not going with SADI?
  14. Hello everyone, I’ve gone through the long process of trying to get a revision surgery after gaining most of the weight back after having the sleeve in 2014. I’ve had mental health issues in the years following and also developed GERD and High blood pressure in the recent years since. The insurance company (BCBS of North Carolina) has been giving me issues from the start first saying that my surgeon wasn’t in network so they wouldn’t cover the surgery. Upon further investigation from my doctors office the real issue was them denying my claim for a revision because there was a lack of evidence showing that I followed proper diet and exercise after my first surgery. Has any experienced something similar? I have the two comorbidities and my bmi is currently 45 so I’m just kinda in shock they denied me especially without even informing me.

    I agree. Four years out from a bypass it’s not very likely that it’s related (could be but less likely). I would give your family provider a call because it does sound like something is off. They will probably want to order labs at the very least.
  16. Good morning I’m going through a bit of a little sad spiel because they denied my revision surgery so my surgeon is going to have a peer to peer. I have Molina care. I’m in Washington Seattle. Do anybody have any type of advice? Will they approve it or am I doomed?
  17. I think the revision has just kickstarted the weight loss again? I'm still pre-op, but I think if you've had the sleeve years ago, the body might be acting as if it's a new surgery? But congrats on the new progress!

    burping? Kink in the sleeve?

    Hello Vets, I've been having increased burping and I'm almost 5Yrs post op. Its been fairly smooth sailing so far in terms of complications (only limited ones not worth mentioning). Now I'm burping uncontrollably. I do not have any any heartburn "typical" symptoms. I'm taking Pantoprazole along with just starting on Carafate. I did not have any GERD prior to surgery however Dr. Aceves told me that I had a hiatal hernia that he fixed during sleeve surgery even though I had no idea I had it. I'm told I may possibly need a revision sleeve to bypass which I do NOT want to do but the new surgeon doctor (I was operated on by Dr. Aceves so I can't see him about this unfortunately, may he RIP... ) told me I may have a "kink in my sleeve". Apparently sometimes they put a stitch or two in to keep the sleeve from floating around. My sleeve was not stitched into place to keep it from flopping around so I guess it may have a kink. I'm supposed to get an endoscopy scheduled in the next few weeks with a GI and the (new) surgeon doc wants to be present to see what she's working with. Has anyone ever had this happen this far after surgery? Confused and concerned...

  23. I had the sleeve done in 2017. I suffered for years with GERD and pain. I had a hiatal hernia repair in April, 2024 and it returned in June 2024. This time my organs were in my chest near my heart. So on November 12th I had a hiatal hernia repair AND gastric bypass. I am sooo tired and run down. The smell of certain foods make me sick. I am so afraid to move to the soft food stage because I am afraid it will cause problems again. I’m taking the vitamins and drinking my fluids. Will all of this pass? I think I am nervous because I wasn’t prepared 100% for the bypass. I knew it was a possibility but had not prepped as a planned bariatric surgery would have been. does the smell of certain foods make sickness go away? I plan to begin the soft diet very slowly . Does anyone have any suggestions? how about the gruesome breath? Does that settle down? thank you for your assistance.
  24. I didn’t have any real taste changes with my sleeve except for with sweet stuff immediately post op but that was only temporary. I am curious to see if it’s the same with my revision. I hope if I do it’s not to any of the healthy things I like since they are already so limited. I never did think to try stuff I didn’t like prior to sleeve though. I guess I should do that after my revision. Maybe I can expand my menu which would be a welcome change.

    How hungry is everyone after gastric bypass surgery? One of my biggest fears is being constantly hungry since no food is actually making it through my stomach. I have a week to go before my surgery, just starting the pre-op diet tomorrow, and a bit nervous if i can make it through without feeling like I'm starving.
