Please help me understand the difference

Biliopancreatic Diversion with Duodenal Switch is the original. SIPS/SADI-S/Loop DS is the new version. The new version was created to mitigate some of the issues with the original i.e the diarrhea issue as well as lessen the malnutrition issue (still higher risk compared to Gastric Bypass, Gastric Sleeve, etc, but better statistics than original DS). However, the downside is you have slightly less calorie and fat malabsorption, statistically slightly less weight loss, and there isn't enough long term data to tell how patients are doing 10-20 years post-op, but that data is available for the original DS. However, statistically SIPS still has a higher weight loss percentage then Gastric Bypass even if it doesn't beat out the original DS. In the end it comes down to 1. if your surgeon even performs the SIPS version of the surgery (which is actually easier for them to perform then the original) 2. if your insurance even covers the SIPS surgery because some insurances consider it 'experimental' due to the lack of long term data and 3. which surgery you actually want. I chose the SIPS/SADI-S/Loop Duodenal Switch for myself because I didn't fancy having to deal with diarrhea all the time. Didn't expect that I'd have to deal with constipation issues at times and my system trying to empty itself at others (at least twice a week my stomach is like "ohhh you're going to be in the bathroom for a WHILE" and I literally empty everything out. Diarrhea still happens on those days. TMI, sorry, but I thought you'd want to know all the nitty gritty stuff to help make your choice). Also, there is something like a 3% you will develop either Celiac Disease or a Wheat allergy. That's for any malabsorbative surgery (DS, SIPS, or Gastric Bypass) and they don't TELL you that. I developed a wheat allergy. I was tested for it after I had classic symptoms of dumping syndrome after I was TOLD that wasn't possible. I tested positive for a wheat allergy. Anyway, it's unlikely it'll happen to you, but I tell people because a 3% chance means 3 out of every 100 people so it's worth mentioning and I'm irritated that they DIDN'T mention it! I still would have had the surgery, but I would have like to KNOW! Also, they didn't tell me that it's possible to vomit old blood after the surgery. Again, doesn't happen to everyone, but it happened to me and scared the SH*T out of me! I was still in the hospital and panicked and the nurse came in took one look at the blood in the bag and was like "It's old blood. You're fine." and then walked out. Like WTF?! Anyway, I'm almost 16 months post-op with the SIPS so if you do decide to go that route feel free to ask me any questions about my own experience and I'll do my best to answer. I also had a lot of pain in the main incision above my belly button- that is where they remove the unneeded stomach and do most of the movement during the surgery. I ended up getting a hematoma. I was fine until 12 days after the surgery my incision opened slightly and lots of old blood starting coming out - it didn’t stop bleeding for over a week. Now my incision is fine and healing well. Sorry if too much TMI but I had no idea that it could happen but I guess it does occur. If the pain gets to be too much - contact your doctor to make sure all is well. Best of luck!

I’m 9 days out and really struggling with pain. I had an incident with TMI pooping today and now my stomach is in so much pain it feels like every move I make I might pass out from pain. -

Checking in on a Friday night! How is everyone doing? I am almost 2 months since my surgery was March 1st. I am eating a little more but still very restricted as far as amount of intake at one time. I have found Quest chips crave my need to "having a crunching sound" when eating. Weight from surgery date is 34 lbs and 54 lbs all together. I have way more energy and each week seems to be better. The hardest part has been the acid reflux and constipation. TMI. If I could figure out those 2 issues, all would be really good! Would love to hear how the rest of you are doing!! -
Positive post for people living with diabetes and in the pre op phase. I became a diabetic at 29 in 2018 my first blood sugar reading was 329 A1C 13.5.I was put on meds metaformin and insulin, but changed my lifestyle and was running too many lows so was taken off. In 2020 I started to get real bad yeast infections( TMI) checked my blood sugar 521 A1C 11.5 scheduled an appointment with a endocrinologist was put on humalog and trujao soloster. After getting my A1C down to 6. I was put on ozempic and trujao. Two weeks after surgery I was told discount all meds and that I didn’t have to see my endocrinologist anymore. Since surgery resting sugar is always 70 never below and after a meal in the 90’s the highest it’s been was 104. Sorry for the long post but I wanted you all to know my struggle. Btw I had surgery 3/3. I hope others who are undergoing surgery due to weight and diabetes get the same blessing!

My sleeve revision to bypass was done in Mexico 4 weeks ago. Performed by the same surgeon who did my sleeve. He was great with the sleeve so I went back. I had lots of post-op pain the days following the procedure. Surgeons went back in to do an endoscopy to check for a leak, but found no leak. They said I was actually constipated (sorry TMI) and likely causing the pain. Once returned hone I developed a post-op infection, went to see a general surgeon who gave me antibiotics for a superficial infection and said that was all I needed. Thankfully it wasn’t a deep infection. I have had so much difficulty getting anything in. I carry Gatorade zero around with me all day, as well as water and protein drink. I’m so thirsty but it hurts to drink much through the day. I do still try. I’m not getting in my protein, but am getting in my Barimelt vitamins! The abdominal pain has increasingly gotten worse over the past week. I’m curled up in a ballin bed, and my gastric area is sensitive to the touch. Any thought/suggestions would be greatly appreciated. Thank you!!

Hello to everyone and a very big thank you to all of you for taking the time to respond. Unfortunately, the guy I was seeing broke it off with me and I never had to worry about bc in the first place (sorry if TMI). It was a pretty upsetting breakup. But at least thanks to all of your input, I have some ideas for the future, if anyone special ever does come along.

Now, I must say, I'm still 250 pounds, so I have at least 60 or 70 pounds to go to get to goal and actually more than that to not be considered overweight for my height, but I was almost 400 pounds, so this a huge difference and I'm feeling amazing. My wedding dress is a size 24. I got married in 2006, and I have not been able to fit into it since 2010. It is now too big! I had to get something notarized the other day and when I got out my driver's license I noticed my weight said 350 and realized I am 100 pounds less than that weight now, which was a lie anyway. Lol That is an amazing feeling. I have never, ever weighed less than what my drivers license said. Even back when I weighed around 150 I never thought I was thin enough and always lied. When I renew it, I will tell the truth, because I accept myself now at any weight. It's been a long journey to get to that point. I got a new dress the other day and thought it needed a belt and the only one I had was from my highest weight and I can wrap it around me twice. It's crazy! This might be dumb, and maybe tmi for some, but my belly was so big before I couldn't use a squatty potty, and now I can. I just bought one and it really helps with my constipation. This is definitely in the TMI category. Look away now and don't read this for the faint of heart! My poop since surgery and delivering those twin rocks 8 days later has been almost BLACK! Not brown but black! I understand why the stool has improved and not as it once was when eating carbs. My carbs are really low right now and that results in a better stool. The color is what is killing me. Must be some red dye in the chewable vitamins maybe? So weird!

Howdy friends I’m 11 days out of sleeve surgery and ever since I left the hospital I have diarrhea, sorry TMI but need help. My doctor said it’s normal but I got like 3-4 times a day and never solid. Has this happened to anyone and please tell me it gets better. Thanks Mark

Acid reflux AFTER Gastric bypass?

I'm sorry that you are having this issue. My SIL has the sleeve, and she has had horrible issues with reflux. She never had it before even with her 3 pregnancies. Before my surgery I had untreated Gerd for 9 years, they discovered it during the waiting period and put me on pepcid. It changed my life! He even went in and had to repair a 2 or 3cm intrusion in my stomach BEFORE doing my RNY. Well everything went great for 6 months, the GERD didn't return- but the loud, explosive (TMI) belching did. So I went back on pepcid and I take it daily. I have heard that RNY has helped several people but for me, it has reduced the acid greatly but NOT eliminated it completely. Hi everyone, sorry, this might be a TMI question. For those of you who have started the pre-op diet, is constipation an issue for you? I have not had a BM in the past few days. It does not feel like regular constipation because I'm not having any bloating or pain or anything unpleasant. I just know that I have not had a BM. And my pre-op diet includes some actual food. I can have one healthy meal for dinner. I have one protein shake for breakfast and one for lunch. Then a meal for dinner. I had a cup of the Smooth Move tea last night in Hope's that this morning I would have a BM, but nope, nothing. Hmmm. I've read about this happening after surgery and I'm mentally ready for that, but I did not read anything about it happening before surgery. Any thoughts? -
I am glad you’re doing well. I am just surprised with the fatigue. I did not know it could get bad. I have chronic fatigue and I hope I bounce back quickly from it. I have a question.. TMI sorry! But are you able to go bathroom ok? I heard constipation is a side effect, also have you had issues with Gerd after surgery?

Oh no. I wonder what's causing the nausea and headaches. You're getting two protein drinks, two lean proteins/meats, and non-starchy veggies in each day? Sounds like a lot of substance for one day. Hope it gets better for you. One more week to go. Then the weeks after surgery of course. Maybe TMI, but no BM the past few days. I'm guessing that's because I'm eating less, maybe. I'm surprised that I'm not experiencing discomfort. Taking a chewable assistance today before bed to hopefully help on the morning. If not, I will crack open the box of recommended tea that I bought for post-surgery constipation that most folks mention. Other than that I have moments of head hunger, but not as much now as I did when starting this pre-op diet process. Funny but not funny, I got a wisdom tooth pulled Friday. So, my desire to try to cheat is limited since my mouth is tender, I can only chew on one side, and I prefer soft foods. Man, this has been good practice for the first few weeks post surgery. I'm only eating soft foods and tiny pieces. I prefer protein drinks because I dont want to trigger pain. Hahhaha Also, do you have a chemical or chalky like taste in your mouth? I kind of is like the.taste you have when trying to swallow a pill but it dissolves some before you swallow. That nasty pill taste. I noticed it Sunday. Very annoying. I keep brushing and using Listerine, but it's there all day until I eat the actual food. Makes water taste disgusting when I'm drinking all day. Shakes are ok. Once I eat real food, I'm fine. But, I dont eat real food untol dinner. So all day water tastes horrible. Can't be keto breath already, right? Seems odd. Sorry for always typing a novel. Hahaha -
I am in purée/ soft food stage (sorta in between) and I noticed a little constipation the other day and dr office said to take fiber so I did all day yesterday but then I didn’t go at all. After surgery they said if you didn’t go for two days it was the start of a big deal is this the same now? Should I just take the milk of magnesia after two days like right afterwards. Is getting enough fiber going to be a lifelong challenge?

So thankful for all these posts! I’m 12 days post op VSG and currently going every 4 days (today was on my own accord without Milk of magnesia) I have history like many of you of constipation. While I’m going, it’s more loose watery with solids mixed. I sure hope this regulates. Sorry if that was TMI; I tend not to be squeamish regarding these things. 😊 -

Oh, that last pound or two are the hardest. So close yet so far. I sat just off my goal for almost three weeks so I feel your pain. It’s like those numbers are just taunting you. Catwoman7 is also right. A real good bowel movement or a night peeing can see the scale drop a pound or so. (Sorry if tmi.) Try to look at your plateau as if your body is just sorting itself out & finding its new ... I so want to say its new normal but that phrase has been ruined forever. I know it’s easy to say that but it is sort of what’s happening. Your body is working out what it needs to function & maintain at a weight in relation to your calorie intake & activity level. It could just be your natural fluctuation around the weight your body is happy at. Mine fluctuates about 1kg (2.2lb) so similar to yours. Congratulations on your weight loss. It’s a wonderful achievement -

Funny enough - the thought of some foods make me nauseous but not all. I don't want to look at another cup of soup or shake! My kids were eating some chicken nuggets and I was like ooooohhhh that looks so good lol I don't even like chicken nuggets - or at least I didn't. I do remember being hungry in the hospital. I don't think I burped that much or even passed flatulence because I had the poop issues - coming out like liquid (TMI). Glad to hear you are doing well over all -

Alcohol during maintenance??

I didn't try alcohol until nearly a year post-surgery. Everything changed. Before surgery, I could drink wine or mixed drinks and develop a lovely mild buzz that would last for about an hour or two. After surgery, I had to give up wine and mixed drinks because of the new effects they had on me. I'd feel nothing for about 30-45 minutes after drinking, then suddenly feel extremely inebriated -- overwhelmed -- and the whooziness would last for several hours, plus sometimes I'd then get a bad headache. And diarrhea (TMI). These days I only drink one can of beer (Guinness Stout) at home. I wouldn't dare drink away from home because of the possibility of severe inebriation. A blood/alcohol card listing weight and BAC showed that hypothetically, a person of my weight would reach a BAC of .10 after drinking just one and one half glasses of wine! If and when you're ready to introduce alcohol back into your life, definitely drink at home first so you can find out how the alcohol now affects you. About a year ago, a woman posted that she drank alcohol at a party and felt completely fine driving home but during the drive she suddenly became overwhelmed by drunkenness and had to pull over and lie down in her seat. She got a DUI. -
I am doing really well honestly. The worst part for me? I have had the poops (sorry TMI). I don't really have a want to eat (probably because more of the bathroom issues lol). I didn't have pain but some mild discomfort. I feel so good - I am 6 days post op. I have to force myself to drink but that is OK. My back is hurting a lot less, the swelling in my legs have gone down tremendously, I am sleeping better - overall I feel great! I do like a mid afternoon cat nap this past week 😊😊😊😊

Hey guys so I am three days postop my surgery was on March 9, 2021 so far the issues that I’m having are a lot of gas pressure bloating any of Domino area I didn’t have a bowel movement until yesterday and it was like straight of diarrhea sorry TMI I also am having hunger like not had hunger like actual hunger or my guts are making that sound so I’m just trying to figure out if that’s normal also every time I have a set of any liquid I hear gas bubbles in my stomach and I burp it’s super uncomfortable/kind of Painful and I just wanted to make sure that this is not like some weird complication any information is helpful
