  1. I've noticed the weight loss is slower after the revision but still moving at a steady clip. I had my revision to bypass in Nov 2021 so still losing but I'm noticing more plateaus and slowdowns, it eventually starts moving again though so no concerns.
  2. After your revision how much extra weight did you loose after? Did it come off slower than the first surgery?
  3. I was sleeved in 2015 and just went through the revision from sleeve to bypass about 3 1/2 weeks ago. So as I'm still in recovery mode I can't really provide a full answer all of your questions. 1. Reason for conversion: I developed a hiatal hernia where part of the sleeve had slipped up through the opening of the diaphragm into the chest cavity, which was causing chest discomfort and only allowing me to eat a couple ounces of food at a meal. After doing an EGD my surgeon was concerned enough to schedule my revision within a week. He said if they just fixed the hernia there was a high likelihood it would recur with the sleeve. If he converted to bypass he said there was a very low likelihood I'd have any further problems. I also had GERD issues from the sleeve which I wanted to address. 2. Post-op period: I'd say for me it was essentially the same. I had the same surgeon and facility which were both great. Recovery plan is the same for both surgeries (clear liquid, full liquid, puree, soft solids, etc.) I had a bit more pain with the bypass but I'm 7 years older than when I had the sleeve. 3. Best and worst part: Can't really answer this yet, except to say it's nice to lose some additional weight (I never hit my goal with the sleeve) but it's tough to have to start over with the recovery. 4. Dumping: Haven't experienced it yet, but I am just up to about 2 to 2.5 oz of soft protein so I have not been able to even try anything that might cause dumping. My thought on the revision is if you are experiencing significant GERD it is definitely worth considering. GERD can cause Barret's esophagus and ultimately lead to esophageal cancer if not kept in check. Having had a relative who died of that cancer in her early 50's I was concerned enough to want to solve it if I could.
  4. I was able to drive a few days after both surgeries (I had sleeve, then revision to bypass). I'm surprised they're sending you home the same day of the surgery however. Regardless, by Monday you should be OK to sit at a desk and answer the phone, just make sure you schedule some time every two hours to walk around a bit.

    Sleeve revisions

    I did sleeve to bypass revision. Same number of incisions, except they had to put a drain in for a week after the bypass. Same level of pain/uncomfortable, which was not a lot in my case. I only took the painkillers for two days and stopped because I didn't need them. I think I felt worse immediately after surgery when I had the bypass, but that may be because I was out for a longer period of time so I was more groggy. That was just in the hospital though, it felt about the same when I got home.

    Sleeve or Bypass? Confused

    While it is indeed standard practice to repair hiatal hernias with a gastric sleeve operation, I don't think any doctor can guarantee you that your hiatal hernia will never come back. The things I've read indicate that the gastric sleeve seems to make your stomach MORE likely to herniate (since a sleeved stomach is narrower and therefore can slide up through the diaphragm hiatus more easily than a normal stomach). I know for a fact that there are some gastric sleeve patients who have had multiple hiatal hernia repairs after the surgery (and others who were told to convert to a bypass because of their hernias). As far as the acid reflux issue, worse acid reflux is a known risk of the gastric sleeve. Some people are lucky and have no issues, but others do develop acid reflux for the first time after the sleeve or have much worse acid reflux after the sleeve. I would definitely ask him about why he thinks that this is not a concern in your situation and what his plan would be for how to deal with it if your acid reflux does get worse after the surgery. (If he tells you that you would just take a PPI for the acid if it gets worse after surgery, keep in mind that PPIs are not enough to control the acid for some people who have really bad acid reflux after the sleeve). You are smart to be thinking about these issues before your surgery. Personally, I deal with both acid reflux and a hiatal hernia after the sleeve (and no I didn't have a hiatal hernia before the sleeve - they looked for it when they did the sleeve). Knowing what I know now, I never would have gotten the sleeve. I think the bypass has stood the test of time better than the sleeve has - after decades of experience with the bypass, surgeons know the risks of the bypass and are better at managing them, whereas problems with the sleeve are often handled by just converting over to the bypass anyway.   7. I’m starting the revision process because of gerd. Can anyone tell me if the pain from the sleeve surgery is comparable to the full bypass??
  8. I know that weight loss with revision is typically slower and somewhat less. Also if you weigh less on surgery day your loss will be less because you don’t have as much to lose.
  9. I had a revision from sleeve to mini bypass over two weeks ago and have followed everything to a "t" including daily walks and have lost like 3lbs!!!! When I had my sleeve 7years ago I was prolly 15 lbs down the first two weeks!!! Can someone say something to make me feel better ❤️‍🩹

  11. Hi! I was sleeved in 2017 and converted to bypass in 2019. 1) Your reasons for conversion from the sleeve to bypass. I wish I did the revision as soon as my symptoms started (GERD) Instead of waiting couple years but I didn’t understand that I was actually experiencing GERD. I just kept trying to rework my diet to see if the symptoms never occurred to me it was something that I could not fit without surgical intervention. 2) What was the post op period like compared with sleeve? It was a easier than when I was sleeved, I don’t know why but I’m grateful 3) What is the best and worst part of having the bypass? To me the bypass is almost exactly like having sleeved, I can’t tell the difference Until I remember the difference in restrictive feelings during eating. With a slave restriction was high in my chest almost near my throat. With the bypass I feel the heaviness disinterest in food. Some days if I eat and dryer proteins source (like grilled chicken breast) I can feel both types of restrictions. 4) Any dumping issues? Yes I do experience dumping although it’s well managed now. I do have a few additional food restrictions than I did it with the sleeve. I cannot eat really sweet things. Or if I do it is literally one bite and I’m done. I rather not feel terrible over food if I can prevent it.
  12. I was sleeved in Dec. 2016 and did extremely well at first - I dropped about 120 lbs in about 6 months, reaching my goal weight. I had some personal/professional issues including losing my job, starting and losing my business, health issues with my kids, etc. and I slacked off and just started eating like crap again and gained almost all the weight back. I had GERD both before and after the surgery but was maintaining it with esomeprazole. I got the revision to bypass in Nov, 2021 and as of today am down a little over 65 lbs. and the GERD is gone. As for your questions: 1) Your reasons for conversion from the sleeve to bypass. - My doctor said I was eligible for a second surgery and that he needed to do an endoscopy to see if the sleeve was dilatated (enlarged). If it was, I could resleeve or switch to bypass. If it wasn't, I'd have to go with bypass. They checked and it was enlarged so I could get either one but he recommended the bypass due to the GERD and potential future complications with long term use of the medication or effects to my esophagus from the GERD. 2) What was the post op period like compared with sleeve? - About the same. I think I felt a little worse right after the surgery because it was longer. My sleeve surgery was 26 minutes, the bypass was about 3 hours. It was supposed to be a little over an hour but there was an issue with one of the stapling devices not being able to reach a certain area so the doctor had to suture by hand which took longer. I believe I felt much groggier and a little more nauseous, but after the first day it was pretty much the same as the first time. 3) What is the best and worst part of having the bypass? - Best part is losing weight again and feeling healthier. Worst part was having to go through the surgery again due to my own screw up. Also this time they put a drain in my stomach for a week which wasn't fun. 4) Any dumping issues? - Not even once.

But often in the middle of the night I wake up, literally chocking on acid from GERD that developed immediately after the sleeve surgery. I am looking to talk to people who have had a sleeve and then converted to bypass.     Had my original surgery 21 years ago. Scheduled for revision on the 21st. Also in the midst of the pre-op diet. Lost 7lbs already lol I hate that my Dr wants me logging my weight every day.

    No mostly just GERD. I never made it to my goal weight with the band, so hoping to lose more weight with the revision. I just had my first appointment about a month ago to start the process, and I have one more visit with the dietician next week and then I see the surgeon for a pre op visit and I'll get my date then. We're thinking it might work out for the end of April tho.

    I have been doing pretty well, overall. Life goes on after surgery for sure. But, I have been blessed. Really my only complaint about the sleeve is that I developed awful GERD immediately after surgery, so I am dependent on Prilosec twice a day. I suppose a revision to bypass would solve that problem.

    March 2022 Surgery Buddies

    From what I understand there are 2 main reasons. 1. If you have a large amount of weight to loose, they will start with the sleeve and when you max out the potential weight loose they can do a revision to bypass and you kind of "start over" and loose a lot more weight than if you just had the sleeve. 2. Acid reflux/GERD. Apparently it is VERY common to end up with it from the sleeve surgery and sometimes it becomes unmanageable which in turn can cause serious damage so they so a revision to bypass to stop the GERD. I am only speaking from, what I have read and what my surgeon explained to me. I have SEVERE GERD so the sleeve was not even an option for me. I am on 2 medications already, and eat tums constantly and have damage to my esophagus already. I need it resolved before I end up with major problems.

    Same, girl. Same! I'm almost a year out from my Lap Band being removed and I've been eating around 600 calories a day. Terrified of regain. I haven't regained while waiting for my revision to bypass tho, thankfully.

    Sleeve or Bypass? Confused

    Sleeve and Lap Band operate under a high pressure system. It puts a lot of pressure on your esophageal sphincter. Some people with these surgeries develop issues with GERD and esophageal motility issues years after having their surgeries. Sure GERD will go away with weight loss, but after years of high pressure on the sphincter, GERD can manifest again. I had a Lap Band for seven years and had it removed a year ago due to GERD and revising to RNY soon.

    What Led To Revision

    I had the sleeve 7 years ago. I lost 90 lbs. I gained back 70. I have terrible Gerd. My surgeon told me a few years out that the sleeve did what it was supposed to do and i won't lose like weight again. I went to a new surgeon and he recommended a revision to bypass. I'm 6 days out. I hope to hell I can maintain this time.

    I have this, it's scar tissue, and more than others because mine was a revision from a band. The eating may place relieving weight on the area. You may want the internal scaring cleared up.

    Stomach capacity

    My gastric bypass was revision surgery from a sleeve due to hiatal hernia and reflux. Since I had the sleeve first, most of the hunger hormone (ghrelin) producing cells were in the part of the stomach they removed. I rarely got really hungry with the sleeve, so perhaps that might carry over with the bypass as well. I know my remnant stomach is still there, but it's a sleeve, not a full stomach so it won't produce the hunger hormone like others. That being said, I do hope after time I can end up eating more reasonable portions. After a year or so with the sleeve I was able to eat 3-4 oz of solid protein with 1/3 to 1/2 cup veggies and a little starch. I am hopeful for similar capacities with the bypass.
