A general gastroenterologist should be fine for starters - with the bypass being the most common current and legacy bariatric procedure, most should have a reasonable amount of experience with cases such as yours, and be familiar with the common maladies (such as marginal ulcers.) They are usually the ones to do an initial EGD (endoscopy) to take a look and see what's going on inside. If necessary, they can then refer you to a specialist interventional endoscopy center, usually associated with a major regional cancer center hospital. Such hospitals usually have a bariatric practice associated with the cancer center, and they are often a good place to go for more unusual cases as they tend to see a wider variety of odd cases; while they, too, tend to be primarily surgeons, they usually have a broader staff of specialists beyond surgeons, though sometimes they may require a referral from a generalist - step one, your general gastroenterologist. Another step you can consider is to try to establish a relationship with a local bariatric practice for annual follow ups - some practices actively encourage this, while others are more cut and done type of practice. My wife and I still see (12 years out for me, 18 for her) our bariatric practice, which is 5-6 hours away, but they have long done telephone consults, but it does serve as a means of keeping them "on retainer" in the event that questions such as this come up - we, or our PCP, can call or email the doc with questions about these types of situations (is this a common thing for your patients, is there something else I should be looking for, etc.?) Check and see if any local practices have support groups open to others beyond their patients as an entry point, and to see if they have a long term interest in their craft.

If you can finish, you absolutely should for the nutrition. Hit that protein goal! You're eating so few calories it doesn't matter! That's my opinion anyway! I'm almost 6 weeks out from bypass. -

I am just going to post an update because this has been kind of my diary thread (never having started an actual weight loss diary). 16 months post op, I have been the same weight for about 6 or 8 weeks now (142lbs) so I think I'm done losing. BMI is 21. I eat about 1500 calories a day and I think I'll have to stay close to that forever (so small compared to what I was eating 16 months ago!!) I can eat a good full cup of food at a sitting. I can chug about 7 or 8 swallows of fluid in one go. Loving my new life. I hope all my November 2021 buddies are too xxx -

I have a sleeve but I have never measured anything. I suspect it might be different for people with a bypass? My stomach tells me when it's had enough - it has increased steadily from a couple of teaspoons 16 months ago to about half a plate of food at each meal these days. Strangely (with hindsight - having read lots here) I was never given any goals at all - just protein first, veg second, carbs if you need something more, and it's worked out really well for me. I hope you can relax into your surgery and best of luck with your new life 🤗 -

I'm almost 3 weeks post op (bypass). I plan to ask my NUT, but how did you measure your food, especially in the early days? Previously, when calorie counting, I would weigh my food. But now when I read the suggested meal plan it says stuff like 3 tablespoons cream of wheat and 2 tablespoons chicken (made that combo up 😅). So I've been measuring by volume because I have a one ounce scoop/or tablespoon. But there is a considerable difference between the volume and weight for most foods, so I'm curious! -
So, I had my rou-y/gastric bypass on May 15, 2018 but have a ton of excess skin now considering I lost 153 pounds. Went from 365 to now 212. But between the lipedema, the rashes/infections/excess skin and being in severe pain from stage 3 lipedema it's becoming hard for me. My legs swell a lot and is becoming a hassle. I do have a GoFundMe because I cannot afford all of the 3 surgeries needed to get my life back. I saved like $8,000 - $10,000 of my own money but it isn't enough for everything and getting lipedema surgeries covered by insurance is very hard or not able to be covered at all. I have medicaid and they don't have much of nothing for lipedema all of my treatments so far I have been paying myself which also have gotten me into debt. Note: GoFundMe is at bottom. @vsgchick I’m glad to hear you reached your goal. I just had gastric bypass revision endoscopic so they reduced my opening from 25mm to 8 and I got two sutures yesterday. I will wait until a month and get on Wegovy as well to increase my weight loss.

Alternative to milk of magnesia?

I had my bypass almost eight years ago and never had a problem absorbing Miralax. And sleevers don't have absorption issues, so it wouldn't be a problem for them, either. So....??? as for milk of magnesia, I usually get the cherry-flavored version. I think I've seen mint-flavored, too. If the Phillip's brand doesn't have it in flavors, the generic brands that most places carry (like CVS or Walgreens) do come in flavors. -
dumping isn't very common in sleevers. About 30% of bypassers dump - so even there, it's not the majority. I don't know what the percentage is in sleevers other than it's a lot lower than 30%. also, plans vary a lot among surgeons. Some are low-carb, some are ultra-low-carb, others don't care about carbs at all and stress balance instead. So yes, some plans would allow potatoes and oatmeal, others would not. The best advice is to follow your own clinic's plan.

Not everyone experiences dumping & fewer do with sleeve than bypass. You may never experience it. Dumping occurs from eating high fat or sugar. Can occur with high processed simple carbs & dairy but less common again. If your shakes are too thick add more water or milk. I used to double the water to my shakes. I cannot stress enough to follow your plan & your staged returned to eating diet. It is in place to protect & support your healing tummy & digestive system. Remember all those sutures & staples holding your tummy together. All because you drank or ate something without an issue doesn’t mean you should. You could have just been lucky. There are several vegans on this forum who successfully lost weight & maintain their lower weight. You just have to be careful to meet your protein & other nutrient goals. There are so many plant based protein alternatives you can choose from. Your dietician should be able to guide you about your options if you have concerns. If eating meat & dairy upset you before surgery they still will after surgery. Are you experiencing real hunger or head hunger? Wanting to eat a specific food or texture or flavour is head hunger. A lot of the area in your tummy that produces the hunger hormone Ghrelin was removed plus because of the nerves that were cut during surgery, the message to say you’re hungry doesn’t get through to your brain or if they do they are often different. And they won’t until you are fully healed. Head hunger is very powerful & is usually what drove us to eating in the past so we think that feeling is real hunger.

Hiatal Hernia surgery before Gastric Bypass?

Hello! I just had my second office appointment after my endoscopy and my surgeon told me since my hiatal hernia is large, he would have to do that surgery first and then a couple of months later he would do the gastric bypass surgery. I was expecting to do both at the same time and now this changes a lot due to time off, healing, etc. I was just wondering if anyone else has had two separate surgeries in their process and how it went? How long did you wait to have the actual gastric surgery? How was HH recovery vs the Gastric? Any information would be greatly appreciated! thank you!! -

Welcome. It’s not always easy to make the decision whether to have surgery or not. Many here have been in the same place. Imagine your life without the need of medication to manage your high blood pressure, a CPAP machine to ensure help you’re breathing & take pressure off your heart at night. You can have your knee replacement surgery & never experience pain or an inability to be as active & mobile as you want. You’ll have so many more options available to you at a lower weight & with better health - no more restrictions or potential limitations on how you want to live your life. I didn’t have any comorbidities at all before my surgery but I knew they very likely were ahead of me. My risk was very high & now it’s extremely low. If you’re worried about the possibility of gerd go the bypass route. Constipation is easily managed & while it can be a regular event while you are losing it improves a lot once you are eating more & a wider variety of foods. Add soluble fibre to your diet & keep in top of any episodes by using a treatment if it persists for more than 3or 4 days. We can give you so many reasons to have the surgery but if you’re not all in or believe you’re being pushed or don’t have any other option, you won’t reap the full benefits of the surgery. The importance of your head in this can not be underestimated - we don’t talk about doing the head work for nothing. But it is your choice. All the best whatever you choose to do. -
Usually they say to boost your metabolism you should exercise more, weight train, sleep more, eat more protein, drink more water, drink green tea, eat spicy food, eat regularly & don’t skip meals. You likely still are drinking lots of water & getting in lots of protein. I’ve been drinking green tea for 20 years - never helped me lose weight. 🤷🏻♀️ You can’t exercise but can you do some upper body weights or resistance band exercises? You could do these sitting down so no strain on your hip or knee. Spicy food would stir up your gerd so that’s a no - it certainly stirs up my reflux. I try to keep my metabolism burning by eating regularly( meals & snacks) which I never did before (was a big meal skipper). I don’t exercise as such just do stretches & use some resistance bands but it all helps. What is it about bypass that specifically concerns you? Maybe some with bypass will share their experiences & alleviate some of your worries about it. Your Gerd does seems quite severe & has already done damage I would think eradicating it would be a good reason to try a bypass. But that’s my thinking.

Wow, very noticeable difference!! Nicely done and I hope you find that visual progress encouraging! 😁 A few thoughts- 1) you've just passed a month since major surgery and your body is working hard to recovery, repair and get you back out there to your active life 🤗 I'm just a couple weeks from my bypass and working on talking to myself/honoring my body, sometimes with humor 😅 but acknowledging "my body is amazing, and strong, look how well I'm recovering", stuff like that. 2) your food plan looks good to me 3) while you can't actually build brand new muscle without being in a calorie surplus, I wouldn't be surprised if getting back to working out after a lull could cause some micro tissue tears and some water retention 4) need new scale batteries? Lastly, I don't know if you'd have access to one somewhere, but a BodPod uses air displacement (something like that) to accurately measure fat vs non-fat. It was so enlightening to me to see that pre -surgery, I legitimately could burn 3000 cals on an active day, even at 5'4 😅 because of my build. Anyhow, I just needed to know that, so when the scale wasn't moving I could say "science!!" If I'm eating 500 cals and burning even just 2000, I will eventually see it on the scale or tape measure. The body is just going to process weight loss and healing in its own sweet time. All that to say, it must really be disheartening to not see the numbers change after all your hard work and I hope you find some answers. Maybe for now frame your loss since Christmas or New Year's, because you've lost so much! Good luck to you! -
Gastrogastric Fistula Repair

Hello, I just had my fistula surgery on 3/13 like you, my original surgery was in 2007. I did awesome for the first 5 years then I started gaining. I just figured I was a failure and accepted it. Fast forward 2022, I have a 9 cm umbilical hernia. Went to UPMC and they told me they wouldn't repair it and go back to where I got the bypass. I went and through all the testing and that's how they found my 2.5 cm fistula. Lot of drama with the insurance company and I ended up having to pay out of my pocket. When I woke up from surgery, I found out it was a bigger mess than they expected. They had to remove a bunch of adhesions, I actually had 2 fistula, my stoma at the bottom of my pouch was completely closed and that part of my intestines were attached to my remnant stomach. Essentially it was like I never had the bypass. They had to remove part of my pouch and intestines and remove almost all of my remnant stomach. Sent from my SM-G973U using BariatricPal mobile app -

Hi Mark, welcome. I noticed that you’re willing to treat other side effects of your weight with medications & tools - CPAP machine, meds for the high blood pressure, and knee surgery for the worn out knees… But you hesitate with GERD and constipation, both of which can be corrected with medications alone (over the counter options as well as prescription). Having the surgery could even correct your high blood pressure and sleep apnea, which would remove those issues from the list. Not to mention, weight loss in the long term is desirable for knee replacement. This leads me to ask, what are you actually scared of? Weight loss surgery is an extremely effective procedure for long term results, and the RNY gastric bypass is highly effective in treating or preventing GERD. I’m so glad you’re here, because there is a lot to learn and reconcile with when it comes to weight in our society. At the end of the day, getting to a healthy weight and keeping it off is what everyone here is working toward. You will never be alone in this if you choose to move ahead. Best of luck to you! -
Hello, thank you for your feedback. I am 5ft 5in. I've considered a revision to bypass. I'm very concerned with the side effects afterwards. I have the Gerd under control for the most part. I stay a way from things that trigger horrible symptoms. I aspirated 3 years ago due to Gerd & almost died from pneumonia. I now have 2 spots on my right lung due to Gerd. They did the upper GI test 4 years & when I eat the food creeps up slowly than goes back down. I eat slowly anyway thank goodness. I don't have dumping syndrome. Do you have any tips on getting my metabolism to work? I can't exercise due to my chronic pain, mobility and balance issues. My left hip & knee is bone on bone. I use a cane to ambulate and can't stand more than 5 min without severe pain. I've been suffering since 45 & things have gotten worse the last 2 years. Now I'm 49 with no quality of life.

So sorry you are experiencing this. I think you need to find another bariatric surgeon & discuss your options. A revision to bypass will help you lose more weight (maybe not a lot but some) so yiu can look into knee replacement surgery in the future & it certainly will help with your gerd. You can not survive on shakes & protein bars. Besides not getting the nutrients your body needs to function effectively, the low calorie diet will be slowing your metabolism down more. You did lose 100lbs. That’s almost a third of yourself. That’s something to celebrate. Remember the average weight loss is about 65% of the weight you have to lose to put you in a healthier weight range. So, not knowing your height or what would be your healthier weight, I’d expect you may have been pretty close to that statistic. So your stall, which are very common & can occur many times, may have been you reaching your new weight set point & stabilising. I find it ironic that your surgeon complained your sleeve was too big. I mean they did the surgery so if the sleeve is too big that’s on them not you.

Just found these two articles about iron malabsorption you may find interesting. Basically it’s more common with bypass than sleeve & with post menopausal woman. Also said one possibility as to the cause is your reduced diet after surgery while you are losing. To absorb iron you need a range of nutrients which you may not be getting. You are eating much smaller portions & many struggle with beef which is considered an easy source of iron. 10 years since Gastric Bypass, and putting weight back on !

Hi I had a gastric bypass just over 10 years ago. I did fantastic and lost over half my initial starting weight. I have kept most of it off but 2stone is creeping back on and I am scared I do not want to go back to that place. Has anyone else had this challenge and what have you done to get back on track ? -

I have completed all my classes, I see the surgeon on Thursday (3/23) then I am told my surgery will be 5-6 weeks out from there, I keep going back and forth on which one to have, Bypass or Sleeve. I have Gerd bad, and its been getting worse, and I am morbidly over weight and need to loose like over 150lbs, so the bypass was recommended, but I hear and have been reading so many stories about dumping and not being able to eat a lot of certain foods anymore and I am not sure I want to live like that the rest of my life. I am a very picky eater all ready and i am worried the things i do like I wont be able to eat at all and then what??? Maybe I am just thinking to much into this, I just don't know but I need to decide in 3 days!! -
I think it is normal to feel scared and worried. Try to relax and occupy yourself with things that you like and hopefully that will take your mind off the surgery. I had my surgery (mini gastric bypass) a year ago and thankfully everything went smoothly with no major complications. This was my first surgery so I went in without any prior experience. I watched some videos on YouTube to be prepared tho. They put a cannula in my arm and it was used for the anesthesia and the IVs afterwards. I was on pain killers for the first few days while in the hospital and the pain was generally tolerable. The only discomfort I felt was because of the drains and I felt much better after they were removed on the third day. Good luck and I hope all goes well with you. Think about your health and how you will look like after the WLS. That always helps

How exciting! It's a big, personal decision. I started my process in January of 23' and my surgery is scheduled for April 24th. I decided on bypass because of my preexisting GERD. I, too, am ready to get healthy.
