    April 2022 Surgery Buddies

This was going to be the topic of my next question. I intended to quote 3 posts but I may have double-quoted one of them by mistake. anyway, my question has to do with the marker BMI and losing weight. I hover between 39.1 and 40 BMI. CIGNA is a stickler about BMI with 35 being a cut off for benefits. I know that I will lose weight when I start the liver reduction diet next week. A good friend who was sleeved in October told me emphatically 'Do not lose any weight.' After the insurance company gives their blessing, is it okay then for me to stop worrying about losing weight. (Such an odd concept)

I agree with @Goldengirl321... I also needed to maintain my BMI prior to the 2 week pre op diet as it was exactly 40. Afraid if I lost weight I would be denied insurance coverage. I lost just over 12 lbs in my pre op diet. You WILL lose weight during it!

    April 2022 Surgery Buddies

Hey there! I am just at a 40 bmi with no co morbidities so my dr actually told me to maintain my weight until surgery. Although I do still have to do the 2 week diet prior and I’m sure I will he said at that point the weight won’t matter. My journey was a little different I did all my pre op testing cardio, pulmonary, nutrition, psych, sleep study and my 12 office visits (per my insurance, Aetna) then I was approved at the end of January. I asked to be scheduled mid April due to some work things I had going on. I now have to do more testing at the hospital on 4/15 an EKG, chest X-ray, Covid test and blood work, I’ll see my primary Mon 4/18 for final clearance and then hospital 4/19. But back to your original question don’t worry too much you will loose in the 2 weeks and be a rockstar!
  4. Golden girl since we are scheduled the same day, I have questions. My preop mtg with dietary snd surgeon is scheduled the first week in April. I finished all my required visits in Oct last year but have been waiting for surgery due to Covid. I am nervous because I haven’t really lost much weight since then. I know I will need to go strict diet for 2 weeks prior to surgery but anyone else who has not lost a lot of weight preop in their own??? I feel like I should’ve done better.

I’m using the App for this question so if it ends up in the wrong place, I’ll do it over in a bit. Odd question…but…I am probably having RNY (or sleeve) with Dr Pilati in Cary NC. Here’s the question…it’s convoluted and long. Sorry. The pre-procedure testing is extensive and I have only have the stress test to do on 3/29. I have a large hiatal hernia diagnosed a week ago. Half of my stomach is in my chest which explains the worsening shortness of breath I’ve been experiencing. EKG normal. I have all the markers for CIGNA to approve my surgery: BMI, HBP, high A1C, high cholesterol, high triglycerides, obstructive sleep apnea. We know the hernia will be repaired during surgery. We KNOW there will be a surgery at least to repair the hernia. WHY the stress test is my question? Does the diagnosis of the hernia preclude the need for a stress test? I will not be able to do 7 minutes on the treadmill at an increasing speed and incline because I cannot breathe enough with the hernia. I’ve done 2 stress tests about 20 and 15 years ago when I was 100 lbs lighter (I was a long distance runner). THEY were Duffy in top shape. I want to ask my surgeon we have to do it but don’t want him to think I’m being a pain in the butt. Opinions/experience on need for stress test in this scenario? Thanks in advance.
  6. Remember it will take weeks (6-8 for me) for your stomach's nerves to heal enough to send you the signals from down there. The restriction will probably be more noticeable later.
  7. Oh yes. The constipation. I was crying when I finally had to go in weeks 2 and 3. Stool softeners helped.
  8. There’s so much conflicting information about it is confusing. I was advised 1/4 - 1/3 cup but this did depend on what the food was (still does almost three years out). I could always eat more soup but less if it was a less liquidy, more solid food like mince or a sausage. Some people can eat larger portions. Others can’t. Generally, tread slowly especially while you’re still healing & listen to your body. It is more challenging at first because the cut & damaged nerves from the surgery mean your old messages of being full etc. don’t work as well & you often find they’re not the same as they were either. Try not to eat until you feel full but until you’ve had enough. That full feeling is actually telling you you’ve eaten too much.

    Negative comments from family members

Trigger Warning: ..............................Yes. My dad is a food pusher. We have a very strained relationship now but growing up he would make 3 different dinners at once and have me eat a bit of all of it and then call me fat and bully me. He use to buy packs and packs of ramen noodles and hide them in my things like my desk/book bag/purse as I started becoming overweight as a child. I still can't eat ramen to this day because it brings me back to those moments. Hed tell me how ugly and fat I was but then push huge amounts of food on me. He's from a country where food is unstable and grew up incredibly poor so when he came to the US and saw how abundant food was he constantly buys food and cooks. He's a big reason why I had a bad relationship to food for so long/why im still working through it and why I still have self esteem issues. He's thin and in shape but has a serious problem with alcohol. I didn't tell him about my surgery because a) he wouldn't understand given a language barrier and he's treated me horribly over the years because I'm fat and the last thing I want is to hear those words but about my surgery. He's picked up on the fact that I had a weight loss procedure done. He tries to mend our relationship but still offers food because he doesn't know how else to show love. He offers me healthy stuff now but I simply just decline because I'm not ready to explain anything to him or become closer. Sent from my SM-G975U using BariatricPal mobile app

Small selection of carefully picked items of clothing that can be mixed & matched to create many more outfits. Often includes those classic separates like well cut trousers, a white shirt, a straight or slightly flared skirt, a blazer, a whit t- shirt, etc. They sometimes say you can include one or two pieces of trend or fad clothing but they still has to work with many of the other items. I’m not a great capsular wardrobe person because I prefer to buy a complete outfit. I find when I do a capsular wardrobe I often can’t wear the piece I want to wear. Like the shirt I want is in the wash as I wore it the day before. Also I get bored easily wearing the same shirts or pants, etc. every week. I could never wear a uniform.

Chi Franciscan - Tacoma, WA thoughts?

I had the duodenal switch by Srikanth in 12/2019. By 7/2021 I was dying of malnutrition. I was taking all my vitamins and drinking my protein shakes - not the pre made ones but the ones you make yourself. I was blacking out, collapsing in parking lots, I couldn’t drive because I was blacking out while I was driving, it got so bad my husband had to bath me. I couldn’t be left alone. I had called Srikanth’s office to schedule an appointment to talk about the symptoms when they first started but was told that I needed to call my PCP so I scheduled an appointment with her. I was put on various medications because my BP was typically around 67/52, I had ear problems in addition to blacking out, collapsing, throwing up any food I could get down IF I could get food down. I eventually ended up in the ER and was admitted for a little over 2 months for malnutrition. Sometimes when you have the duodenal switch your stomach doesn’t absorb enough nutrients and your body will start shutting down. It was awful. Now I’m on TPN and have my vitamins, proteins, carbohydrates, and electrolytes pumped through my blood stream 12 hours a day. I’ve had sepsis 3 times in 2 months. I’m not saying it happens to everyone but the nurses in the hospital said they see this all the time. No one mentioned this before surgery. No one helped me after surgery. Srikanth’s office told me to go see my PCP who is not a specialist in weight loss surgery. I’m so dissatisfied with Srikanth’s office with the exception of Karli. She is wonderful but my issues came on before she started working there but now I have a resentment against there office and don’t ever want to go back. It MAY not happen to you BUT watch out for those symptoms.
  13. Can you tolerate raw veggies to the point of salads yet? I'm craving a big crisp salad, but it will be weeks before I can attempt that. I was going to suggest tempeh, but WhispersOfAngel already did. Here's a salad that sounds delicious to me: https://www.bonappetit.com/recipe/warm-chestnut-and-apple-salad

Yeah, it's not clear yet and it was a new guideline from my dietician post-op. She switched me to eating every 2 hours to avoid nausea. Initially, I tried to eat my meals just the 3 times a day and found I'd get sick at night. She thinks I need to take in less amount of food more frequently instead of more less frequently. It's worked so far, I'm not getting sick from eating but it's hard to get my water in too. The meals I have, I usually eat them in parts. Like I'll have half of it and then the other half later so I am taking in small amounts of food all day. They also want me to cool it on the protein shakes because they think it's causing my a lot of constipation. I started making my own because I couldn't do all the milk products so now I just have one a day and get the rest through food. I def think you're right though... I need to drink more water and hopefully, some of the painful movements will get better. Water is soooo difficult though but I'm determined. Still trying to figure out the right balance for me.
  16. …and just like that, almost 600 calories in my tum in no time flat! chicharron (i.e., pork cracklings) are very sliderish for me, kinda like chips. I ate all of this save for 3-4 pieces that someone pinched from my bowl when i left it unattended lol 594 calories:

    Any pre op smokers

How kind of you to say! It's going pretty well so far (three weeks in).

    Any pre op smokers

Yes the addiction is so real, was not planning on quitting but was forced to. My mom was a smoker for several years and to quit you just have to be ready and I was not there yet, still am not but Im being good for the sake of surgery. I see you had your surgery a couple weeks ago hope all is going well.
  19. I have NO IDEA how you're supposed to meet your water goal when you can't drink for 40+ minutes (because you need to count eating time too!) out of every 2 hours. That's bonkers. I had surgery 2/16 so I'm a similar ways out as you, and my plan has me eat 1/4 cup of protein for breakfast, lunch, and dinner, and then supplement with a protein shake/powders to hit my protein goal. It helps a LOT to hit protein and hydration goals when you can drink part of your protein! You said "I eat 3 small meals a day, one snack and a Protein Shake" so that shouldn't be every 2 hours, right? Like I do 8 am breakfast, 12 pm lunch, 6 pm dinner, and a protein shake somewhere in there and sometimes another half a scoop of protein powder in there if my meals weren't high enough protein. Even with a snack, you should have a solid 4 hours or so between most of your meals, right? If they really want you eating every 2 hours, see if you can replace some of those with protein shakes? That's yikes! For the constipation, I'm sorry you're dealing with that! It's likely because you're not getting enough water - those stool softeners you listed work by pulling water into the stool to soften it, but if you're not getting adequate hydration all they're doing is bulking it up and not actually softening it.

    February 2022 Surgery Buddies

I had mine on Feb 14th so I'm 5 weeks and 3 days out. I felt pretty good about fluids and protein until like 2 weeks ago drinking water and feeling pain/restriction, especially when it's room temp. Using a straw and having ice water seems to help me get it down better. I actually gulp in a lot of air when I don't use a straw. Most teams tell you not to, even mine but I feel gas pain in my chest without the straw. I'm doing really well with protein though and get 70-80 grams a day through about 700-800 calories. Rarely do I get to 800 cals though. I noticed on days when I have eaten up to 800 cals I feel nauseated at night so I try to not go over 750. It's a small difference but weird enough I can really feel it. I eat 3 small meals a day, one snack and a protein shake. My dietician has me eat 2 oz of protein every 2 hours during the daytime. I'm taking vitamins well, no hair loss...yet? I do need to take nausea medicine in the morning because maybe 4 times a week I wake up with the instinct to dry heave. I don't know why but I've always felt nauseous in the mornings, even pre op. I find that I can either focus on food or water but not both. When I drink 64 oz of fluids, I eat less and miss my protein goal. When I reach my protein goal, I drink less fluids. I find that balance to be really difficult to deal with and get right. I wonder when that'll be easier? It's also tough with the 2-hour eating rule and then the 10 min before 30 after drinking rule. I'd basically always have to have food or fluid in my mouth all day which isn't realistic for me. I've lost 29 lbs in 5 weeks. I don't really feel it yet/still feel like a big out-of-shape lump LOL. I think I'll feel the weight loss when I hit the 240's. I have my one-month post-op tomorrow with my nurse practitioner. Looking forward to it. P.S. Is anyone still dealing with constipation and ROCK HARD stool? I am on prune juice, Metamucil, colace and occasional senna tea. I go almost every day but it's incredibly forced, usually bleeding occurs or I need to slap on a pair of gloves and take it out (I know TMI I'm sorry ) . I'm going to talk to my team about it tomorrow but I'm so done with it. It also causes me a great deal of nausea. Wondering if anyone else is dealing with this and what you're doing to make it better?
  21. Hey everyone I had surgery 2/8 so I’m at 6 weeks post op I’m still only able to eat two small meals and I drink a protein shake to make up for the last meal and still no snacks. I am drinking atleast 5-7 water bottles a day so I’m also hitting my water take. It takes time soon enough you all will start to feel more and more normal atleast for your new stomach.
  22. My surgery was on 16/2. It’s been 5 weeks post surgery. My fluid intake has gone up so much and I’m starting to lose weight again now. On the advice of my surgeon and dietician and trial and error, I’ve done these things: 1. Track my water usage using the Waterllama app. it’s cute and easy to use 2. track my food intake using the “see what you eat” app, making note of only the proteins I consume (I don’t count macros otherwise. Just make sure everything is “light” or “Low fat” and mostly “sugar free”) 3. set reminders on my phone to take medications, eat and drink (I don’t feel thirsty or hungry so this has been dangerous for me) 4. use water enhancers and uncaffeinated herbal teas to vary the taste of my fluids I haven’t been hospitalised other than that 1 night post surgery however I’ve been making weekly trips to meet the care team. I’ve lost 25lbs, which isn’t much. I was barely losing any in the previous 2 weeks but according to the body analysis, I was losing muscle and water. I was lethargic all the time but could still move. Once I got the proteins and fluid settled, I’ve been feeling so much better. For movement, I searched for anything “post bariatric surgery” and was doing basic stretches and walking for 15min then moved to 30 min. Now I look for “Low impact/intensity beginners cardio” lasting for about 30min to 45min. Don’t push yourself to the point of pain. Once I feel twinges, I stop. The physiotherapist in the care team suggests that movement be done for 10-15 min, twice a day at a start. For the first 3 weeks, my heart rate doesn’t go beyond 65% of my max heart rate. Now I’m pushing it to about 80%. After week 6 I will incorporate weights again. I enjoy resistance training and can’t weight to get back to it so just do what feels right to you. Find the type of movement that you enjoy. Whilst my body has been changing even without exercise, I am constantly reminding myself this is the same body. WLS is not magic. I have to take care of my body before I can get it to work right. This means eating drinking moving and resting sufficiently. I’ve been on 5 weeks of leave and this has helped me with my new-habit forming as well as allow me to heal gently. I will finally get back to work next Monday. I don’t know how I will be energy and pain wise but we’ll see.
  23. So I have gotten different answers from my doctor's office (nutritionist, doctor, post-surgery team, etc.). I am meal prepping on the weekends and I portion my food into small 1/4 cup portion cups, that are typically used for dressings and condiments (I LOVE THESE THINGS!). Anyway, I noticed that at times, I can eat 2 of these within an hour (making it 1/2 cup), while other times I really can only have one of them before I feel restriction. I am so worried about damaging my stomach or busting a suture. What amounts of food have you guys been able to tolerate at 1 month post op? Also, bear in mind that my doctor's office moved me from liquid to pureed after week 1... then from pureed to soft food on week 3. I am on soft food for another couple of weeks before it's venturing out to regular food. Is there a good rule of thumb in the bariatric world to follow? Like maybe only eat 1/4 cup for each "meal" and have 4-5 "meals" per day? I always get the answer "it's variable and depends on the patient." I think I'd prefer some type of structure to go by. Also, my doctor said I wouldn't have to worry about dumping because I had the sleeve, and it's very uncommon; however, I have noticed that if I eat too fast, or don't wait 45-60 mins to drink after eating, that I get diarrhea. It's short-lived thank God, but it still is hard to prevent (which makes for a pretty uncomfortable environment at work!!) It sounds like dumping, but he said it's common... thoughts?

Hello! I had surgery on 2/22 and had major complications afterwards. I was in the hospital for 13 days out of the last month. I, too, had a hard time getting protein and fluids down and my weight loss has also stalled at 16 lbs. My doctor ordered thrice-weekly fluid and vitamin infusions for me. Perhaps you can ask your doctor about that.
