    Revision sleeve to bypass

    I am getting a revision done this coming Wednesday. What happen was I had my sleeve done in nov 2013 and I did great! But I had very bad heartburn so I started taking nexium daily. In 2016 my surgeon did a hiatal hernia repair. That did not help so I just dealt with it and ended being put on dexilant medication for Gerd. So I had a upper GI and endoscopy and it showed regurgitation and acid reflux . So my doctor sent everything over to blue shield of California PPO. And it was authorized by them . My surgery was supposed to Take place April 8, 2020 but had to be rescheduled because of the coronavirus. So my new date is this coming Wednesday, June 24 and I got my clearance and I even had the Covid test last Thursday so I’ve been quarantine till Wednesday. I’ve also gained some weight in the last two years so this should help with my Gerd and weight gain. I’m very nervous but I’ll be so happy to be off the medication for the horrible heartburn.
  2. Your doing well, dont worry about how much others lose .. My loss was slow in the beginning and i had my doc telling me my loss was slow compared to others however they had been only sleeved.. Not revised to sleeve, so to me he was comparing oranges to apples! From what i have read generally, revised to sleeve patients lose weight slower in the beginning then they catch up, and this is what's happening to me. Just a lil advice take your measurements at least once every couple of weeks, this will show you while weight isnt coming off the scales, that its possibly shifting. Just stay on the path and keep plugging away. It will start to come off Wishing you every success with your weight loss journey xox

    Oh so much to talk about with this..... Sorry, but I'm going to ramble a bit. First off, there are risks and benefits to each of the three surgeries (as background I had a lap-band, miserable with it, prolapsed, lead to emergency removal, and now have a gastric bypass. I'm also a family practice physician assistant. I'm not a bariatric expert, but I've been around this a bit). On average, lap-band patients do not lose as much as the other 2 surgeries. That is why surgeons typically don't recommend lap-band above a certain weight. That said, I know people that have lost 200+ pounds with the lap-band. Average weight loss is just that, an average. For everyone that loses 10%, there's someone that loses 80% of their excess weight. It's impossible to predict where you'll fall in that spectrum. Some of it is how good your surgeon is, some of it is how dedicated you are to lifestyle change, some of it is sheer luck. As far as the band, I won't say don't do it. I will say research your butt off. Again, weight loss tends to not be as much as compared to the other 2 surgeries. I would like to add that, while the band is known for it's reversibility, that is not entirely an accurate statement. My personal story was that my band prolapsed, stomach herniated (and before i get jumped for being a bad band patient, I wasn't a bad band patient....sometime crap just happens). I had an emergency removal and was supposed to revise to gastric bypass, however, once the surgeon got in to remove the band I had so much swelling and inflammation that he was not able to do the revision. They removed my band, sewed me up, sent me home to heal for 6 months, then I had my revision. I know of at least 2 individuals in my area that had band removal and had so much damage that they could not revise to another surgery....while the band is removable, I don't really feel the term reversible is 100% accurate. Another point I'd like to share is 'invasive' truth be told, you're having major surgery. All 3 surgeries are invasive. The sleeve does not have the malabsorption issues or dumping associated with the RNY. However, they are essentially cutting off a large section of your stomach. The RNY does have malabsorption and possibly dumping (most people don't dump, I happen to be in the minority that dump easily, it's really not a huge deal if I watch what I eat). Yes they are making a small pouch in your stomach and bypassing and small portion of your intestine, you are getting some replumbing done with the RNY. That said, RNY is reversible (again, subject to scar tissue and such associated with previous surgeries) IF you need reversal. The vast majority of us will never consider reversal because we need this surgery to maintain a healthy weight.....(reversibility was not a factor in my choosing RNY). I know people that have been hugely successful with all 3 surgeries. I know people that have lost almost nothing with all 3 surgeries. I know people that have had bad complications, hospitalizations, ICU admissions with all 3 surgeries. None of them are magic. None of them work without a commitment from the patient. It is not something to step into lightly..... My advice, research, research, research. Find a surgeon that you trust and listen to what they say and what they suggest. Ultimately you have to make a decision that you are comfortable with and that you can commit to. But know that every day you spend morbidly obese is taking a toll on your body and your health. Most of us just cannot lose an maintain a healthy weight without surgery. Most of us have lost and gained a thousand times. There are some that regret the surgery but the vast majority of us wish we'd done it sooner. The RNY is right for me, I wish i'd done it in the first place. But I'm not you. I wish you the best of luck with the decision and applaud your choice to start considering surgery. If you ever have questions I'm happy to try to help. Best of luck with this decision and congrats on trying I take control of your health. It's a tough choice but at least you're considering it. That's a step in the right direction.

    Update: Revision surgery

    Hi Tulip, I was banded by Dr. Marsden on Oct 19th. Now I'm all freaked out about my port flipping.. YIKES! I guess time will tell, huh? I wonder if he used the dissolvable stitches on me, or if he used regular. Any idea when he switched? I'm glad the revision surgery went well - hopefully you'll be up and around in no time!   6. Hi everyone! I was originally sleeved in March of 2016. I had an annoying burping thing pretty much from the beginning, but I lost about 90lbs in 6 months...and have held pretty steady since. Then the extreme reflux started! Fast forward to yesterday....I spoke with my surgeon and it has been decided that I need to be revised to an rny. Definitely not happy about it, but I’ve been taking 3 nexium a day and still having reflux and regurgitation. The newest issue appears to be an ulcer. This is really the last thing I wanted to do, but I’m very worried about the damage that’s being done to my esophagus. I have developed a raspy voice and scratchy throat, so I know it’s becoming a problem. It looks like I’ll have surgery either in March or April....I’ll probably know in a week or so. Anyone else here a revision from VSG to RNY??

    Does anyone still get "stuck"?

    The sleeve isn't supposed to cause that to happen, but I have heard that sometimes there is scar tissue left over after lap band removal that still gives you restriction as if the band were still there. I would definitely talk to your surgeon about what you are experiencing and see if s/he wants to investigate why this is happening to you before your revision.
  9. I had the band 6 years ago. I lost initially but then never found a good spot, either to tight or to loose. I'm working towards revising to the sleeve (hopefully late February). Thing is, my band is basically empty and I still have "stuck" episodes. I feel like I always get stuck if I am out to eat. So I want to know if people still get stuck with the sleeve? I know the same rules apply and you have to chew, but I want to know. I am so sick of excusing myself to the bathroom (only my immediate family knows I have the band) Sent from my iPad using the BariatricPal App

    Does anyone still get "stuck"?

    I used to throw up literally every time I ate. I would panicky if we weren't seated close enough to a bathroom at a resteraunt, because I knew I'd be in there 3 or 4 times. I hated eating with other people outside my immediate family. I'm only 21 days post op right now, but I know I had a pressure/pain feeling everyday in my chest for the last couple of years and now it's just gone. I had such severe scarring that it took 5 hours to do my revision surgery and was warned I may need the area where my band was dilated in a few months. However no pain in my chest and even though I'm just to mushy foods I haven't had that stuck feeling even once, and I used to get it with water sometimes. Band coming out was the best choice for me!
  11. I would agree. I'd revise to a DS. You probably need that surgery. I don't think you will be happy with the RNY and think you will likely not get to your goal weight with it. I can say this because I have RNY (virgin surgery) and have a larger capacity. So I do not feel a whole lot of restriction. And food goes through my stoma very easily. So I have to be extremely careful of what goes in my mouth, calorie, volume count and limit food in the face of hunger. And I got to my goal. But it took iron balls to do it. And not everyone can put that kind of limit on themselves. If you haven't already, you need to have your sleeve evaluated to see if they left too much of the fundus in tact, or if it can be revised to make the sleeve smaller in a re-surgery. If it's surgeon error, they should have to fix it. If it's your error through overeating or eating around restriction, then a new surgery won't necessary fix your head and you will need to consider counseling along with a new anatomy if you want to avoid having this same discussion in the future.

    Dr Ramos-Kelly-Revisions?

    I would also be interested in this information. I'm considering Dr. Ramos-Kelly through Successfully Slimmer (Christi) instead of Dr. Zapata through Ready4Achange who coordinated my band surgery in 2005 with Dr. Sanchez. I had a phenomenal experience back then, but have no history with Dr. Zapata and am not familiar with the new hospital they're using so I'm not sold on going through r4ac again. Anyways, I would love to hear any information about Dr. Ramos-Kelly both with first time surgeries and revisions. It sounds like Sandy and Christi are coordinators with different LLCs, but Trish is actually an assistant to the doctor. Is this true? Thanks everyone!
  13. I know im having mixed emotions too. I am not good with pain. I dont like pain meds maybe the liquid iv one lol. The one I will take at home I dont care for too much. I will ask him in the morning if he would switch me. Actually I had the bypass on July 17. Revision was on July 19. He will look to discharge me tomorrow after my primary doctor sees me iamready4this

    Considering revision

    A revision won't cure mindless eating (I guess you're already well aware of this) but it can help with reflux issues. I got a revision to bypass myself and reflux issues got way better. If you're having reflux issues a surgeon will usually advice against a sleeve.

    Considering revision

    I was banded in 2011 and lost 80 pounds never quite got to my goal weight before I started having problems. Throwing up multiple times daily, bad acid reflux to the point I was needing 8 pillows to prop me up to sleep, frustrated. I was too tight at one point from the swelling from vomiting so much, that I had to get all fluid removed emergently so I could swallow my own saliva. I ended up getting all fluid removed and esophageal manometry done. I have a separate post about that experience. The manometry showed I had a slip. That was the documentation I needed in order to get a revision for mybinsurance to cover my revision. There needs to be documentation to support another surgery need. Unless you are cash pay. I initially wanted sleeve because I thought it was the best option....however after speaking with my surgeon, he told me that I would more than likely have problems with severe reflux due to the pressure from the sleeve and since I was already miserable from the severe reflux I had, I wasn't about to have to deal with any more. I also have several friends who had sleeve done and have significant regain with reflux issues. He said revising to RNY would resolve reflux and I wouldn't ever have to deal with that issue. I decided that was the best choice for me. I was revised to RNY back in 2015 and SO Happy! No reflux which is awesome! I can eat spicier foods again. I have lost a total of 125 pounds and at my goal weight of 175. I am also a Zumba Instructor and teach Zumba 4 days per week. I keep my protein first, then veggies and fruit. I take my vitamins daily and actually have not been sick as much either since revision. I have not had any issues. Love my RNY ❤ I only wished I had done RNY to start with. Do your homework, ask questions and decide what is right for you. Best of luck to you!

    I'm 400 lbs...will this still work for me?

    The studies have been posted numerous times on obesityhelp.com Since RNY was not an option for me, because I knew of the complications from real life friends, I never saved any of the links. They also talk about only 30% of RNY patients dumping with sugars and carbs. The weight regain stats with RNY are downright scary, and you can search the Revision and RNY board over there to see just how many people are looking to get revisions called ERNY (extended rny) which is where they shorten the common channel even more to make the body start malabsorbing calories/fats again.

    Vsg revision to bypass

    not sure about on this forum (although I'm guessing there are some on here), but there are lots of people who have had a sleeve to bypass revision (mostly due to GERD). Most of them I know of were glad they had it done since it resolved their GERD. Weight loss is quite a bit slower with a revision, though - but it can still be done.
  18. So I Had My Gastric Sleeve Done IN June Of 2013 I STARTED Out At 327 An Got Down To 244 AS Of March 2014. I was hitting many obstacles an had a injury at work which made it to were i couldn't work out. I had many stalls an no longer loose weight an actually started gaining an now up to 271. After talking to my husband i sometimes think i never got the sleeve done the reason being is comparing everything to his. I never had the dye test done an wasn't loosing weight good. I was pretty much starving myself an was hungry all the time an even now when i eat i can eat a normal meal like someone that hasn't had it done. I emailed the doctors office that performed it because i now want some type of proof it was done. If it was done i wanna no why he left my sleeve big enough for me to not be able to get great weight loss like others an why did i not have the dye test done or they never called me for follow ups. Has anyone had anything simular like this happen to them an reconsidering a revision?
  19. my surgeon told me that after my revision, as I was at goal weight with a slipped band everything is great now except that I loved the restriction feeling as I don't have that anymore, I'm only 8 weeks out and I'm looking forward to restriction in future possibly January I've been told.

    Band to Sleeve patients lose less?

    I had my band to sleeve revision on 8/28/14. I have only lost 22 lbs and have been stalled for the past 6 weeks. I am extremely frustrated. I consume 90-100 grams of protein, 60-80 carbs, and 1200-1300 calories. I have been walking 1-2 miles, several times a week. I am starting to feel like a failure, or that I am not doing something right. Original SW before band was 275. Lost down to 190. I was 220 when I had the revision done. My goal is 150. It is starting to feel like I have gone through this surgery for nothing.

    I'm addicted to sugar...

    MPW09, I too had a sweet tooth and also had type 2 diabetes and on the lap band was able to keep eating sweets as the healthier foods never would go down for me and I would PB. My diabetes worsened with the lap band because of that. I didnt know all the junk food would still go through the band I only thought something like ice cream or sugar drinks would be an issue. I'm much older than you and was at the point where I couldn't even lose weight anymore, forget the keep it off part and due to my health I had the lap band removed and eight months later revised to RNY. I'm fairly new at it and know the effect sugar will have on me now and it's not pleasant. I also don't crave it like I used to (knock on wood)when I was banded and before and am off my diabetic meds completely. I know it's not a "popular" view here at LBT but thought you might as well hear the other side of it from someone also. Good luck to you which ever surgery you choose. Nancy.

    Need Advice

    Even though it will be a tough conversation, and she probably won't want to hear what you have to say, you need to talk to her about getting professional help from a psychologist and a nutritionist. All of the revisions in the world aren't going to matter one bit if she doesn't change her relationship with food. The surgery allows for weight loss during the "honeymoon phase" pretty much regardless of one's eating habits. That changes soon. The majority of this journey is about the mental game. If she's eating all of that crap on a regular basis, she's losing that battle. Good luck. I know this isn't easy, but I'd definitely have that conversation before she puts her body (and your checkbook) through more trauma.

    Get my band out

    I got my band in Dec 09 and I lost 40+lbs but then nothing. Band was tight bc like u I would puke probably 3-4 meals a week. At 211 I just felt like I failed though much healthier than 260. I went to Mexico for a revision to gastric sleeve. The sleeve surgery was 5k and I paid an extra $500 for band removal. Best decision! I am 6 weeks or so out and down to 186 pounds. Good luck whatever u decide!
  25. Hi, I am six days post op and feeling okay (except a little dizzy). I still have over a week of liquid diet let per doctor's plan--- but getting so sick of the sweet protein shake options. I have ordered a bunch of protein soups. Hope everyone one in August is doing well!
