    Sleeve or Lap Band?

    My family has a history of Diabetes on both sides of my family. My Uncle lost both legs, another a foot, my father his kidneys. All were adult onset, eventually insulin dependent. I've had a long struggle with weight loss, hypertension, etc. and was on a trajectory to see Diabetes in my near future. For me the band was a consideration, but not an option. It would not be permanent, it's mechanical, can fail, needs maintenance and would eventually go away. Not to knock it but it is not for me. The benefit of the sleeve is that you don't have the Vitamin dependencies as with others, it is permanent and is a positive control. You know you can only eat so much, hopefully you won't exceed that frequently, and there is feedback if you eat too much. As the sleeve can later be revised to a duodenal switch it provides another stepping stone, should you need it. I reviewed all of the options with my surgeon well before hand. I've had friends - with diabetes - who have had a full bypass leave insulin, reduced their BP, etc. - but the switch was the best option. I'm now 13 days past the operation (first couple of days, not the easiest) but generally getting around, drinking and eating a little. You may question your decision after you have it done but you are not alone in that. Good luck!

    @@VSGAnn2014 Im sure it does unless something else is happening, than by all means correct me. I will be referring to my Aunt who failed her gastric Bypass. They went in and did a revision and she quoted the surgeon who told her her stomach stretched to a "flabby grape fruit". So maybe its different case by case. She had a Gastric Bypass but was able to Finish a LumberJacks Morning special which is easily 3-4 cups of food compared to the 1-2 ounces shes SUPPOSE to eat. How is that possible if it cannot stretch?
  3. You and I could be twins! I had a VBG done in 2000. It was difficult to get insurance approval for the conversion to a RNY, but this year, I was finally approved. Funny thing, my staple line was also broken and food/liquid would go through the staple line and just sit there and eventually I would throw up food that I ate days before. That section of my stomach would blow up like a balloon, put pressure on my lungs and I couldn't breathe. Twice I was rushed to the hospital with suspected heart attacks. When a fat old lady can't breath, they automatically assume it's a heart attack. I had horrible GERD, hiatal hernia, sleep apnea, diabetes, and obesity-related asthma and insurance said a revision to a RNY wasn't medically necessary. My surgeon convinced the insurance company to approve me because of my GERD. Ok....I'm off my soap box now. The first surgeon I saw in 2010 told me that I would have only lost 50 pounds when I was denied by insurance. I guess this was his way of telling me the grapes were sour. My current surgeon said that there is no reason why I can't lose 100+ pounds. After eight weeks, I've lost 58 pounds. I had to have two upper endoscopies because the surgeon needed to know my stomach anatomy. I have no clue as to why I had to have two, I just followed his instructions like a robot. I wanted this surgery so bad. I had to have a swallow x-ray. Because my surgeon had no idea about how much scar tissue he would find when he started the RNY surgery laproscopically, I was his only surgery that day because he said he might have to open me up to complete the surgery. I went into the surgery not knowing how long it would take or how long I would be in the hospital. I consider myself very lucky that the procedure was done laproscopically and I had a fantastic recovery. My surgeon's team advised me to lose FIVE pounds before surgery. From everything I had read, almost all doctors wanted at least 10%. I lost the 10%. I wasn't taking any chances on anything happening. Because I started eating healthy, exercised at least an hour every day, and stayed very very active up until the day before surgery, my recovery was so much easier. Good luck with your pre-op. It will all be worth it in the end.
  4. I wish I was pressured by my doctor when I first had WLS 15 years ago.I should of gone with the RNY.I went with the lapband.Fast forward....I am seeking revision to the RNY.

    Approvals with BCBS Federal

    I think the surgeons office tries to collect sufficient information for approval process. I know with my revision my office want proof that I complied to previous surgery. Luckily i had all my post op nut visits with measurements and weigh ins. It was not a requirement but helped in the process.

    Band to sleeve...gaining weight

    this surgery is tricky to say the least. I too am a band to sleeve revision....now putting on...proteins Water exercise....maybe not drinking enough water...hmm...increased exercise a lot...but i am having sugar in tea...in India we drink a lot of tea....i think sugar is totally banned for us.. our body is now refusing to cooperate...like it has a mind of its own.

    High BMI's

    My BMI is 57, the last time I was weighed I tipped the scale at 409, and I'm 5'11". I decided to do the sleeve after doing lots of research. I have talked with several people who have lost over 200 lbs with just the sleeve. I have talked to those who had the bypass revision after being sleeved. I made the personal choice to go with the sleeve because I fear some of the effects of the bypass. If I need to do it in the future, it will still be a possibility, but I would like to try without the need of rerouting my gastric system.

  9. Nope - I picked a surgeon/surgical practice with great results and trusted his judgment. They try to do what the actual research supports and in 2011 there was not a clear benefit to a "smaller" sleeve and there were indications of higher risks. I was a band to sleeve revision which is already higher risk so it made sense to me. If you take a look at the info on "bougie" - search this site and google it - you will see there isn't that much difference in end stomach size. Thank you ellie, it has recently occured to me that alot of my info is quite dated as i am no longer researching. As new studies come out, even my practice changes recommendations. For example, in 2011 they said to get 60g of Protein, at my last check up they said minimum of 80g. They base it on research that shows people are more successful at that higher level. My disclaimer is that i am only an expert on myself and my experiences.

I would love to know how it goes for you. What prompted your revision? I did email Alex to ask to a sleeve revision board. It seems like there are more of us. I will keep you posted. I could really use someone to talk to through this ross. I have my husbands full support but my family is not supportive at all. I am having the revision because of very severe acid and bile reflux. Medication does not manage it. I wake up every night vomiting and choking.

  13. I am doing my surgery with Dr. Fox. He's been featured on CNN etc and is supposed to be very good with tons of experience under his belt. The surgeon who did my lapband referred me to Dr. Fox for the revision, in my surgeons world "I can do your revision, but Dr. Fox is WAY better than I am at it"...

    Arm Lift Revision

    HAve any of you ever been on the site realself? I typed in the arm revision in search bar and got lots of good feedback that comes from doctors (at least let's hope) =Having a secondary arm lift, or brachioplasty, is not as uncommon as you would think. Further weight loss, under-correction are common factors that lead us to consider secondary brachioplasty. The "flaps" are actually more hearty after the primary procedure and there is usually less to do, resulting in a more spot-on post-op result that my patients love. =Yes, it is possible to have a second arm lift performed. It's not uncommon in our weight loss patients that they have skin that is poorly elasticized - that it tends to stretch out easily, despite a properly performed initial surgery. And if you have lost more weight, this may also be contributing to the looseness that you see. More skin excision might help the situation. Speak to your local surgeon about this idea. =You can have a second arm lift or brachioplasty. After massive weight loss, the skin looses its elasticity. After body contouring procedures, sometimes it is necessary to revise the procedures or re-excise the excess skin which develops over time. From El Paso, Las Cruces, Mexico.
  16. In 2001 when I had my lap band placed, I remember my Mom asking why I couldn't just pretend I had the band and eat like that. Bless her, she was serious. It was her pull yourself up by the bootstrap attitude. (I can't hardly bend enough to reach them these days, let alone pull myself up). After much explaining, and more so after the surgery was done, she came around and was a big supporter. Mom passed away a year and a half ago, but I know she would support me in my revision to a sleeve (even if it had to be after I had it done to get there). Best wishes to you. I'm glad you have a plan in place.

      @proudgrammy, my 5 month surgiversary would have been in 2013. I have posted my sleeve story on my page but in a nut shell my sleeve is angulated or twisted. I also have horrible Gerd. All this developed post pregnancy. I have had to have endoscopy and barium swallow and my surgeon believes best course of action is revision. Also my weight loss stalled @Threetimesacharm 10/21 is my day.

  19. Here is an alternate view. I had to remove my band due to complications also. It was removed in april and my sleeve was done in June. During that brief time I gained a huge amount of weight. i do believe that weight loss is slower and restriction is less as a revision patient. For example I could easily eat 1500 or more calories now at 3 months out. I do not know why that is but the sleeve is working for me because I am full very quickly but I don't have any problems eating anything. I don't know the answer to your question but please don't let yourself gain back too much weight. I also lost 100% of my excess weight and am now back to square 1 (although I am losing weight steadily). It just REALLY sucks to do this again!

    My GNY Story

    I had bypass in 1983. Long story short....I'm at 280 again... I am having bypass revision on Sept. 29. The best advise I can give you is this..the first time I had this done I thought it was a miracle. This time I understand that it is a tool. That IM responsible for doing what's right not the surgery. If you follow the "rules" and keep yourself on track...it will be the best thing you ever do for yourself..

    I had the same situation. I am having a bypass revision Sept. 29. I'm so excited because the first bypass I was young and thought out was a miracle....this time I know its a tool and that I am responsible for my success not the surgery...its just there as a boost and to help me.

    Gastric bypass vs sleeve

    I chose the bypass for two primary reasons. 1. - I had read about waaaaay too many people have a sleeve to bypass revision and only wanted to do this once. 2. - It was my understanding that with the sleeve you were still able to tolerate sweets and with the bypass they were a lot less tolerated. I know myself when it comes to sweets and I am never satisfied with one of anything! Therefore, I did not want to put myself in a position where I was "allowed" to have sweets because I knew I would not be in control of the situation. Since my surgery in June, I have attempted sweets/sugar twice and dumped both times!

    Arm Lift Revision

    I personally have had an arm lift in January 2013 and had great results. I have had other plastic surgery where I feel like you that they did not remove enough skin in the first place. Having consulted with another PS he did say that with bariatric patients you really have to be aggressive when removing the skin. It looks like your PS was not aggressive enough. If you can go to someone else I would, otherwise definitely I would go for a revision.
