Stricture, Malnourishment, and TPN

Hey all! About a month out from surgery I went in because I had an unusual series of events happen. Out of nowhere, I was able to eat significantly less than I had been (just yogurt and cottage cheese), and I had even vomited after a few occasions of simple drinking (Water only). So, I went in and had a swallow study done. That confirmed my new stomach was slow to empty, and my surgeon suggest a scope to go in and verify and if needed, dilate the closing connection to allow me to eat. I got called the next day, offering me a time on Monday to go in, or I could wait about two weeks. Since my symptoms had gotten better by this time, I said I didn't need to rush in and get it done, and that I could wait for two weeks. That was another big mistake. I found my symptoms would be ok in the morning and then at night, they'd flare up and I'd end up vomiting and being unable to keep anything down. This eventually morphed in to being able to consume no Protein shakes (I was put back on Clear liquids and protein by my doc regardless of when my procedure was, UNTIL I had my procedure.) I felt fine though because my body was still burning fat. But all things being the same, I put a note in my online chart for my nurses to see Monday, asking for the next available date to do the scope. Monday I was called and told to come in Tuesday. Everything seemed very routine, and the overwhelming feeling was this procedure would fix my issue and I'd be back on track. Wrong! I got up after the scope ready to walk out and feeling refreshed from what I'd just call a nap. The nurses had different plans for me. As they wheeled me back to recovery, I asked where my wife was. They told me she had gone to get something to eat and would be up soon, in the meantime, I should order some food. I was told that I could "have whatever I wanted," which I immediately questioned. I also asked why I needed a meal if I was leaving shortly. The reply was that I wasn't leaving shortly, I was being admitted overnight. I was literally shocked. Confused, and now wishing my wife was there to answer some questions, I tried to get whatever I could out of the nursing staff. They had limited information because they're all just staff in surgical recovery, they don't know all the details of every patient, and there were a lot of them. I got ahold of my wife who told me she had actually gone home because they said it would be 2 hours before I'd even get back to a room. She told me the dilation was only mildly successful, and that I was malnourished and had to stay overnight for observation. Long story short, I ended up with a PICC Line, a massive bag of TPN, and orders for home health to help me with future daily bags of TPN for about the next 4-6 weeks until they can go back in and do another scope. I'm feeling a lot of things right now, most of that is disappointment that this hasn't gone as easily as really everyone led me to believe. I understand every patient is different, but it's so hard to hear things like "you're an ideal candidate for this now because you're so healthy and your only comorbidity is hypertension, so you're mostly healthy aside from that," or, "we don't anticipate any complications in your case." The moral here is if you think you're having issues, call your surgical team ASAP and tell them. I have no clue if there was anything I could have done to avoid this, I honestly doubt it, but here I am with a 2 liter bag of Fluid I'll be toting around for the next month and a half either way. -
Hello all; I'm 5'3 HW 245 SW 235 CW 211. I'm 1 month out and cant seem to find a variety of soft foods. I'm so tired of eggs, beans and yogurt. Any suggestions?

I am 19 days post op and Im eating tuna, yogurts, eggs, any kind of soups, and of course shakes. -
I traveled out of my area for surgery - my surgeon approved soft scrambled eggs and yogurt immediately. My local doctor, however, is much more strict with her patients. So I actually regressed when I got home and followed my local doctor's instruction of liquids, then puree's. Did it suck? Yes. But I said I would follow her plan and that's what I needed to do. I know it's tough and you want to eat regular food, but you need to follow your doc/nut plan. Just because someone else's doc says to eat food immediately, doesn't mean that's the right thing for everyone.

What brand of protein powder? I usePhD ? The Belgium choc I'd delicious and I bought pb2- peanut butter powder to mix with it...tastes like a Reese peeves and ups protein intake...Greek yogurt...cheese string...refried beans...add non flavour to a protein shake you have already made?

Hey everyone, so I am here feeling a little fustrated. I am 10 days post op and I am supposed to be still on a full liquid diet until monday. Well today I was able to eat 8oz Protein shake 4oc Cream Soup 4oz Greek yogurt (plain) 2 string cheese Then again 4oz of cream soup And 4 oz ricotta cheese I feel like maybe this is too much to soon?!? I know I wasn't supposed to have the string cheese and ricotta... How was able to fit all this in though? I am calling my nutritionist Tomorrow and ask if that's normal. What do you all think?

I would stick to your protein drinks until you know for sure. My plan includes veggies, oatmeal, applesauce, yogurt, jello and pudding. Also to have 4 protein drinks, 2 with milk and 2 with another liquid. -

Being Vegan its kind of hard to get a high number of Cal. in but I'm getting between 400-600 I try hard to reach as close to 600 as possible. I do not take in any sugar of any kind and I do not snack at all. I actually have an alarm set for my meal times. I have not gotten in fruit yet because I do get dumping syndrome and I'm actually scared fruit will be to sweet...for anyone who has not gotten that..OH MAN :-/ I think the closest I get to sweet is unsweet soy vanilla yogurt and that seems to settle well with me. I have actually started lifting weights today, not to much but just trying to tone. But when that scale says I gained it actually makes me cry..I did not have a good experience with my WLS so to not lose is heart breaking. -
I ate Greek yogurts, soups, applesauce, puréed sweet potatoes, cauliflower mash, black beans, avocado. I was able to handle over easy egg and ate that too. Hope that helps.

Hi DivineRN, What really helped me were my Premier protein shakes about 30g of protein per serving really helped me get a good amount in quickly. I also kept a large bottle of water with a natural protein flavor packet with me all day and sipped as much as I could! Lastly, greek yogurt, cottage cheese and pureed chicken pot pie were go to's. Hope had helps! The scale moving kept me pretty motivated during that time.

@@Kathy Coop I find it better to avoid it like herpes. Hollow calories, no reason to eat it. Splenda works fine in my coffee. Asides from coffee and yogurt I do not eat anything sweet. I signed up for a lifestyle change and I am going to stick to it. Weight loss has been amazing this past month, nearly a pound a day. My NUT will faint on the 6th when I see her. -

Had my 2 week post up today. Doc said im ahead of the curve doing better than they expected. I still have 2 weeks left of full liquids. It's funny im so hungry when i see pizza or burgers or real food, but when it comes to my meals, my yogurts and stuff im just not hungry. Lol swelling has definitely gone down, doing better with gent fluids in. Still horrible at protein though. I mean less than 20 grans a day. I just can't do any more shakes or powders. I have to get better with it i know. Im going back to work tomorrow. Incisions healing nicely. No complications, nausea or anything like that. But expecting to experience that once i start real food or mushies. I really want eggs and cheese and avocados. Lol -

@@Kathy Coop I have come close 3 times now. I have a firm understanding of what is okay, and what is too much. And by close I mean one more bite, and I would have been gone. I got sweats and a rapid heartbeat. A bit of nausea, and the desire to lay down, but no vomiting or volcano ass! Phew! I do not imbibe sugar, however some things contain natural sugar, like an apple. 1/2 an apple is my limit. The yogurt I eat Oikos Triple Zero has Stevia in it, which apparently the body thinks is sugar, one I can get away with, two and I am right on the border. Plus full, so I never went for a third, lol. Dumping as I see it is a blessing. I CANNOT eat sugar. That is a good thing. Although my NUT said the oddest thing to me last time I saw her. She said after a year or so, dumping is not as bad, you might be able to eat like a 1/2 a cupcake. I nearly exploded at her. I was like do you tell everyone who comes in here to eat cupcakes after a year? WTF is wrong with you? She tried backpedaling, but by then I was all over her. I nearly stormed out of her office and complained to the staff that this moron is telling RNY GB that Cupcakes are okay after a year. I was FUMING. She is new and I told the desk staff to never schedule me with her again. -

As a rule (pre-surgery) I don't like refried Beans but they're packed with Protein and I add some shredded cheese and light sour cream and I can eat them. It's a break from cottage cheese and yogurt. I whip the cottage cheese with some sugar free blackberry jam and it tastes better. I also do ricotta instead of cottage cheese sometimes. I am at almost 6 weeks and yesterday was the first day I could hold down scrambled eggs. I had turkey sausage with it and ate that first and I think since is was higher in fat it smoothed the way down for the eggs. I still can't eat chicken, tuna comes right up. Crab with a little mayo stays down and so does lobster. The softer the food the better. I struggle to get to 60 grams of protein daily but found a 30g unflavored protein powder that I add to the refried bean or to stewed lentils today that got me up to the right amount. The brand is GENEPRO Medical Grade Protein which I purchased on Amazon. -
I am officially 3 weeks out today(sleeved 12/08/15). I am having a very difficult time keeping anything other than Greek yogurt and cottage cheese down. I have also been eating small amounts of sliced cheese because my new tummy can tolerate it and I'm so hungry. I am taking Omeprazole and don't have any typical reflux, I eat very slow(chew, chew, chew and time my meals out according to surgeon), I stop drinking 30 minutes before, nothing to drink during, and nothing 30 minutes after eating. I am starting to feel like a bulimic throwing up everything I eat. I am going to be calling my surgeon tomorrow because I don't want to wait until my one month on the 8th. Any suggestions? Anyone else experience this? I am not even close to my protein goals with just the Greek yogurt and cottage cheese. I am managing to get in my water. I told my surgeon I was throwing up at my two week follow up, but at that point I was only on puréeds and only really having problems with eggs, which he said is a very common issue with eggs. I am on soft foods if I didn't mention that before, but if I can't literally put my fork through it, I don't eat it. This is not fun!

I feel much better now...except I can't eat yogurt or my protein shakes. I haven't got much protein since the surgery.

I'm doing okay, I can't keep the protein shake down. I just tried some yogurt with 12g of protein in it and my sleeve did not agree. How about you?

My usuals are either a protein shake, 1 egg with a little bit of cheese or a yogurt. It all depends how im feeling. I usualy need to wait at least an hour before I can eat something. -

Fear-Not making it through the educational program the Nut has instituted before surgery. Other fears post-op I try not to think about until insurance approves a date. I am struggling with staying on plan. There are days I do just fine with 10 grams or more Protein per meal, and under 10 grams each of sugar and fat per meal. But others I am still feeling hungry and wanting to eat more. I'm sure it is a head game, but is this normal? Especially before my cycle, I couldn't keep meals under the fat and sugar content (aversion roughly 15 grams each). I eat a lot of Greek yogurt so my protein can be above 80 sometimes when I have pork or chicken in the same day. Meeting with the Nut after one month next week. I had two bad days over Christmas and regained what I lost so focusing hard this week to get back on track. I hear after surgery you aren't hungry so I am hoping this is normal and just the hormone aspect of hunger that is gone after the sleeve? I also REALLY struggle with slowing down. Taking 10 minutes to eat a yogurt is really slow-but the longest I've taken for a meal is 30 minutes. And I was forcing myself to do that-not sure why that is so hard. -
Vegetarian/Vegan RNY Oct. 2014 surgery

I've been a vegetarian (no eggs or fish) for the past 30 years. Plenty of ways to get protein: tofu (I bake or saute it till it's crisp and using a dipping sauce of soy, ginger and rice wine), lentils (soup or curried), vegetarian chili (crumble tofu), chick pea salad, pea soup, veggie burgers, etc. There's Protein bread, protein Snacks (100 calories/pack and 19 grams of protein), high protein Cereal,( soy or milk based-- lf cottage cheese, lf cheese, nf yogurt, etc) Protein is protein. The difference with some plant based Proteins is they can also be starches. In the case of Beans, a serving would count as one protein and one starch. -

I hate yogurt but really like the Dannon Light & Fit Greek that comes in cherry chocolate, or raspberry chocolate. I can almost pretend its cheesecake. Only 80 calories and 12 grams of Protein. Yum-o! -
Has anyone found the journey after surgery easy?

Absolutely not. I'm currently in the Stage 2 diet meaning I can eat cottage cheese, yogurt, soups, etc in 1-2 oz sittings. I have found that since I can now eat some solid food I want to resume eating more. The 1-2 oz fills me up but I find I'm having to exercise a great deal of restraint with not wanting to "nibble" on other proteins. For now the carbs aren't a huge issue for me as I've done Atkins before with significant success. -

I had my surgery on 12/21, I came home 12/23. To say I am stubborn would be an understatement, therefore I never pushed the pump but once. I was up walking as soon as they would let me out of bed, pushing myself as far as I could go. Setting my alarm to make sure I was working my liquids etc. My shoulder pain ended up moving to my chest and making it very difficult to breath about 3AM. The nurse turned off my alarm and handed me pain pills and said trust us we know what we are doing. She was the best thing that could have happened to me. Heat made a huge difference with the gas pains and even though the doctor kept me an extra night it was worth it. I went home feeling great, I only have had to use the pain meds at night. Now let's talk the liquid diet, I can get a Protein shake and a protein hot chocolate down. I cannot stand chicken broth in any form, never had that problem. I can eat Jello but everything is just so sweet. I finally tried tomato Soup and liked it. Tomorrow I'm going to try yogurt and sugar free pudding. I am really looking forward to soft foods starting next Monday after my check. I gained 7 pounds in the hospital that was fluids and that has been off after about 3 days. I am so thankful for my Dr and nurses who helped me understand that I could be driven and not be in pain.
